Description
Overview of the medicine
Coralmax-D 500 mg Tablet is a nutritional supplement containing elemental calcium from coral sources and Vitamin D3 (Cholecalciferol). It is primarily used to maintain healthy bones, prevent osteoporosis, and treat conditions associated with calcium and Vitamin D deficiency.
Uses & Indications
Dosage
Adults
Typically, 1-2 tablets daily, preferably with meals, or as directed by a physician.
Elderly
Same as adults, with careful monitoring for renal function and hypercalcemia.
Renal_impairment
Use with caution. Dosage adjustment may be required; consult a physician. Avoid in severe renal impairment.
How to Take
For oral use. Tablets should be swallowed whole with water, preferably after a meal to enhance absorption and reduce gastric upset.
Mechanism of Action
Calcium is an essential mineral for bone formation and maintenance, nerve transmission, muscle contraction, and blood coagulation. Vitamin D3 facilitates the absorption of calcium and phosphate from the gastrointestinal tract and regulates their metabolism, thereby promoting bone mineralization.
Pharmacokinetics
Onset
Clinical effects on bone mineral density are gradual, usually observed after weeks to months of consistent use.
Excretion
Calcium: Primarily excreted in feces, with a smaller amount via urine. Vitamin D3: Excreted via bile and feces.
Half life
Calcium: Varies depending on physiological needs; Vitamin D3: Approximately 15-20 days, but its metabolites can have longer half-lives.
Absorption
Calcium carbonate is absorbed from the gastrointestinal tract, with approximately 25-35% of elemental calcium being absorbed. Absorption is enhanced by Vitamin D. Vitamin D3 is well absorbed from the gastrointestinal tract in the presence of bile salts.
Metabolism
Calcium is primarily not metabolized but regulated. Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D (calcifediol) and then in the kidneys to 1,25-dihydroxyvitamin D (calcitriol), the active form.
Side Effects
Contraindications
- Hypersensitivity to any component
- Hypercalcemia (high blood calcium levels)
- Severe hypercalciuria (excessive calcium in urine)
- Nephrolithiasis (kidney stones)
- Severe renal impairment
- Hyperparathyroidism
Drug Interactions
Bisphosphonates
Calcium can reduce absorption. Administer at least 30 minutes apart.
Corticosteroids
May decrease calcium absorption.
Thyroid hormones
Calcium may reduce absorption of levothyroxine. Take at least 4 hours apart.
Cardiac glycosides
Hypercalcemia may increase the risk of cardiac arrhythmias.
Thiazide diuretics
May increase the risk of hypercalcemia.
Tetracyclines & Quinolones
Calcium can reduce the absorption of these antibiotics. Take calcium at least 2 hours before or 4-6 hours after.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia (nausea, vomiting, constipation, thirst, polyuria, muscle weakness, bone pain, confusion). Treatment involves discontinuing the supplement, hydration, and medical management to lower calcium levels. Seek immediate medical attention.
Pregnancy & Lactation
Considered safe and often recommended during pregnancy and lactation to meet increased calcium and Vitamin D requirements. However, always consult a physician before use.
