Description
Overview of the medicine
Corazon 25 mg Tablet is a beta-blocker used to treat high blood pressure, angina (chest pain), and certain heart rhythm disorders. It works by blocking the action of certain natural chemicals in your body, like epinephrine, on the heart and blood vessels.
Uses & Indications
Dosage
Adults
Initial dose 25 mg once daily; may be increased gradually up to 100-200 mg daily, divided into 1-2 doses as per physician's advice.
Elderly
Lower initial doses (e.g., 12.5 mg) may be considered, with gradual titration based on response and tolerability.
Renal_impairment
No dosage adjustment is generally required for mild to moderate renal impairment. For severe impairment, caution is advised.
How to Take
Take the tablet orally with water, preferably at the same time each day. It can be taken with or without food, but consistency is recommended.
Mechanism of Action
Corazon selectively blocks beta-1 adrenergic receptors, primarily in the heart. This reduces heart rate, myocardial contractility, and cardiac output, leading to a decrease in blood pressure and myocardial oxygen demand.
Pharmacokinetics
Onset
Within 1 hour (oral)
Excretion
Mainly excreted by the kidneys (approximately 95%), with a small amount unchanged.
Half life
3-7 hours
Absorption
Well absorbed from the gastrointestinal tract, though subject to first-pass metabolism.
Metabolism
Extensively metabolized in the liver by CYP2D6.
Side Effects
Contraindications
- Severe bradycardia (slow heart rate)
- Second or third-degree heart block (without a pacemaker)
- Cardiogenic shock
- Decompensated heart failure
- Severe peripheral arterial disease
- History of severe bronchial asthma or severe chronic obstructive pulmonary disease (COPD)
Drug Interactions
Fingolimod
Increased risk of severe bradycardia and heart block.
Other Antihypertensives
Potentiation of hypotensive effect.
NSAIDs (e.g., Ibuprofen)
May reduce the antihypertensive effect of Corazon.
Calcium Channel Blockers (e.g., Verapamil, Diltiazem)
Increased risk of bradycardia, hypotension, and heart block.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe bradycardia, hypotension, cardiac insufficiency, and bronchospasm. Management involves symptomatic and supportive care, including atropine for bradycardia, glucagon, and vasopressors for hypotension.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Beta-blockers can cross the placenta and may cause bradycardia and hypoglycemia in the neonate. Excreted in breast milk; caution is advised during breastfeeding.
