Overview of the medicine
Albendazole is an anthelmintic medication used to treat a variety of parasitic worm infestations, including hydatid disease, neurocysticercosis, ascariasis, hookworm, pinworm, and whipworm. It works by preventing the worms from absorbing sugar (glucose), which is vital for their survival.
Uses & Indications
Dosage
Adults
For common intestinal worms (Ascariasis, Hookworm, Pinworm, Whipworm): 400 mg as a single dose. For Strongyloidiasis/Taeniasis: 400 mg once daily for 3 days. For Hydatid disease/Neurocysticercosis: 400 mg twice daily for specific durations as per physician's advice.
Elderly
No specific dose adjustment is generally required, but caution is advised in those with pre-existing hepatic or renal impairment.
Renal_impairment
No dosage adjustment is required for mild to moderate renal impairment. Use with caution in severe renal impairment as the active metabolite is primarily excreted via bile.
How to Take
The tablet should be thoroughly chewed or crushed and swallowed with water. For optimal absorption, it is recommended to take it with a meal, especially one with fatty content.
Mechanism of Action
Albendazole acts by inhibiting microtubule polymerization in parasitic helminths. This leads to impaired glucose uptake, depletion of glycogen stores, and a decrease in adenosine triphosphate (ATP) production, ultimately immobilizing and killing the parasite.

Onset
Onset of action varies depending on the indication and parasite type, typically requiring several days of treatment for some systemic infections.
Excretion
Primarily excreted via bile as the sulfoxide metabolite. A small amount is excreted renally.
Half life
Albendazole itself has a short half-life. Its active metabolite, albendazole sulfoxide, has a plasma half-life of approximately 8-12 hours.
Absorption
Albendazole is poorly absorbed from the gastrointestinal tract. Absorption is significantly increased when taken with a fatty meal.
Metabolism
Extensively metabolized in the liver to its primary active metabolite, albendazole sulfoxide, and then to albendazole sulfone and other inactive metabolites.
Side Effects
Contraindications

Drug Interactions
Theophylline
Albendazole may increase theophylline levels; monitoring of serum theophylline levels is recommended.
Cimetidine, Dexamethasone, Praziquantel
These drugs may increase plasma concentrations of albendazole sulfoxide, potentially leading to increased side effects.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. There is no specific antidote for albendazole overdose.
Pregnancy & Lactation
Pregnancy Category C. Albendazole should be avoided during the first trimester of pregnancy. It should only be used if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk; caution should be exercised when administered to a breastfeeding woman.
