Cosmoxim 200 mg Capsule contains Cefixime Trihydrate, a third-generation cephalosporin antibiotic used to treat a wide range of bacterial infections. It is effective against various Gram-positive and Gram-negative bacteria.
Uses & Indications
Dosage
Adults
Usually 200-400 mg once daily or 200 mg twice daily, depending on the severity of the infection. For uncomplicated gonorrhea, a single 400 mg dose is recommended.
Elderly
No specific dose adjustment is required unless renal function is significantly impaired.
Renal_impairment
For creatinine clearance 21-60 mL/min, dose should be 75% of the standard dose. For creatinine clearance <20 mL/min, dose should be 50% of the standard dose.
How to Take
Cosmoxim 200 mg Capsule should be taken orally. It can be taken with or without food. For best absorption and to minimize gastrointestinal upset, it is often recommended to take with food.
Mechanism of Action
Cefixime is a bactericidal antibiotic that acts by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved 2-6 hours after oral administration.
Excretion
Primarily excreted unchanged in the urine (about 50-60%) and feces (about 10%).
Half life
Plasma elimination half-life is typically 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Absorption may be slightly delayed but not significantly reduced by food.
Metabolism
Limited metabolism; approximately 10% is metabolized in the liver.

Contraindications
- Known hypersensitivity to Cefixime, other cephalosporins, or any component of the formulation.
- History of severe hypersensitivity reaction to penicillin (due to potential cross-reactivity).
Drug Interactions
Probenecid
May increase cefixime plasma concentrations by decreasing renal tubular secretion.
Carbamazepine
Concomitant administration may increase carbamazepine levels; therapeutic drug monitoring may be necessary.
Warfarin and other anticoagulants
May increase prothrombin time (PT) and INR, increasing the risk of bleeding. Close monitoring is advised.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
