How to Take
Swallow the tablet whole, do not crush or chew. Take at least one hour before a meal, preferably in the morning.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that works by irreversibly binding to the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells, thereby inhibiting the final step in gastric acid secretion.
Pharmacokinetics

Onset of acid suppression within 1 hour; full effect within 3-4 days.
Excretion
Primarily via urine (80%) and feces (20%).
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration, bioavailability is about 50-68%.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 enzyme system, primarily by CYP2C19 and CYP3A4.

- Monitor Vitamin B12 levels for prolonged therapy (>3 years).
Doctor Notes
- Advise patients on lifestyle modifications to complement PPI therapy.
- Counsel about potential risks with long-term use (e.g., C. difficile, fractures, hypomagnesemia, B12 deficiency).
- Re-evaluate the need for continued PPI therapy periodically, considering the lowest effective dose for the shortest duration necessary.
- Consider drug interactions with clopidogrel and warfarin.
