Description
Overview of the medicine
Cinnarizine is an antihistamine and calcium channel blocker primarily used for motion sickness, vertigo, dizziness, and Meniere's disease. This 5 mg suspension is particularly suitable for children or patients who have difficulty swallowing tablets.
Uses & Indications
Dosage
Adults
For motion sickness: 15-30 mg (3-6 teaspoonfuls if 5 mg/5 mL) 2 hours before travel, then 15 mg (3 teaspoonfuls) every 8 hours during travel. For vertigo/Meniere's disease: 30 mg (6 teaspoonfuls) three times daily, or as directed by physician. Maximum recommended daily dose is 225 mg.
Elderly
Elderly patients should use with caution; lower doses may be appropriate. Increased risk of extrapyramidal symptoms with prolonged use. Consult physician.
Renal_impairment
No specific dose adjustment is generally required, but caution is advised in severe impairment. Consult physician.
How to Take
For oral use. Shake the bottle well before each use. Measure the dose accurately using the provided measuring spoon or cup. Can be taken with or without food, but taking with food may reduce gastrointestinal upset.
Mechanism of Action
Cinnarizine inhibits calcium influx into vestibular sensory cells, thereby reducing vestibular excitability. It also has antihistaminic (H1-receptor antagonist) and anticholinergic properties, contributing to its anti-vertigo and anti-emetic effects.
Pharmacokinetics
Onset
Varies, typically within 1 hour for anti-vertigo effects, but full therapeutic effect may take longer.
Excretion
Excreted mainly via feces (approximately two-thirds) and urine (approximately one-third), primarily as metabolites.
Half life
Approximately 4 hours, though reported values range from 3 to 6 hours.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentrations are typically reached within 1 to 3 hours after oral administration.
Metabolism
Extensively metabolized in the liver via N-dealkylation and aromatic hydroxylation, primarily by CYP2D6.
Side Effects
Contraindications
- Hypersensitivity to Cinnarizine or any other component of the formulation.
- Acute porphyria.
- Known allergy to other antihistamines with similar chemical structure.
Drug Interactions
Antihypertensives
May potentiate the hypotensive effects of antihypertensive drugs.
Anticholinergic drugs
May increase anticholinergic side effects (e.g., dry mouth, blurred vision).
CNS depressants (e.g., alcohol, hypnotics, sedatives, tranquilizers)
May potentiate the sedative effects of Cinnarizine.
Storage
Store below 30°C in a dry place, protected from light. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, stupor, ataxia, vomiting, and extrapyramidal symptoms. Management is symptomatic and supportive, including gastric lavage and administration of activated charcoal, if appropriate.
Pregnancy & Lactation
Pregnancy: Limited data available. Use only if clearly indicated and the potential benefits outweigh the risks. Consult your doctor. Lactation: Cinnarizine may be excreted in breast milk. Not recommended during breastfeeding due to potential for sedation in the infant. Consult physician.

Lab Monitoring
- No routine lab monitoring is generally required. However, in patients with pre-existing liver impairment or during prolonged use, liver function tests may be considered.
Doctor Notes
- Advise patients about potential CNS depressant effects, including drowsiness, and to avoid concurrent alcohol use.
- Exercise caution when prescribing to elderly patients or those with Parkinson's disease due to increased risk of side effects.
- Educate patients on the importance of accurate measurement and shaking the suspension well before each dose.
