Description
Overview of the medicine
Coughex 45 mg Tablet is an antitussive medication used to relieve cough associated with common cold, bronchitis, and other respiratory conditions. It works by suppressing the cough reflex in the brain.
Uses & Indications
Dosage
Adults
Typically 10-30 mg every 4-8 hours. For 45 mg, it might be a sustained-release formulation taken once or twice daily, or for more severe cough, consult a physician.
Elderly
No specific dose adjustment generally needed, but start with lower doses and monitor.
Renal_impairment
Use with caution in severe renal impairment. Dose reduction may be necessary.
How to Take
Take orally with or without food. Do not exceed recommended dosage. Swallow the tablet whole; do not crush, chew, or break.
Mechanism of Action
Dextromethorphan acts centrally on the cough center in the medulla to elevate the cough threshold, thereby suppressing the cough reflex. It is a non-opioid antitussive.
Pharmacokinetics
Onset
15-30 minutes
Excretion
Mainly excreted renally as unchanged drug and metabolites.
Half life
2-4 hours (for parent drug); 11-13 hours (for active metabolite, dextrorphan)
Absorption
Rapidly absorbed from the gastrointestinal tract.
Metabolism
Extensively metabolized in the liver by CYP2D6 to its active metabolite dextrorphan.
Side Effects
Contraindications
- Hypersensitivity to dextromethorphan or any component.
- Concurrent use or within 14 days of discontinuing MAO inhibitors.
- Children under 4 years (for some formulations, may vary).
Drug Interactions
SSRIs/SNRIs
Increased risk of serotonin syndrome.
MAO Inhibitors
Increased risk of serotonin syndrome (hyperpyrexia, muscle rigidity, mental status changes).
CYP2D6 Inhibitors (e.g., Quinidine, Fluoxetine)
May increase dextromethorphan levels, leading to increased side effects.
Storage
Store in a cool, dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, dizziness, drowsiness, ataxia, nystagmus, CNS depression, and respiratory depression. Severe overdose can lead to serotonin syndrome. Treatment is supportive; Naloxone may reverse CNS and respiratory depression.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed. Excreted into breast milk, use with caution during lactation.
