Overview of the medicine
Coxia 90 mg Tablet contains Etoricoxib, a selective cyclooxygenase-2 (COX-2) inhibitor, used for relieving pain and inflammation in conditions like osteoarthritis, rheumatoid arthritis, and acute gouty arthritis.
Uses & Indications
Dosage
Adults
Osteoarthritis: 30 mg or 60 mg once daily. Rheumatoid Arthritis/Ankylosing Spondylitis: 60 mg or 90 mg once daily. Acute Gouty Arthritis: 120 mg once daily for a maximum of 8 days. Acute Pain/Primary Dysmenorrhea: 90 mg or 120 mg once daily for acute conditions, for shortest duration.
Elderly
No dose adjustment is generally necessary, but caution is advised due to potential age-related decrease in renal and hepatic function.
Renal_impairment
For mild renal impairment, no dose adjustment. For moderate renal impairment, a maximum of 60 mg once daily is recommended. Not recommended for severe renal impairment.
How to Take
Take orally, with or without food. It can be taken once daily. Swallow the tablet whole with water.
Mechanism of Action
Etoricoxib selectively inhibits the cyclooxygenase-2 (COX-2) enzyme, which is responsible for the synthesis of prostaglandins involved in pain, inflammation, and fever. By inhibiting COX-2, it reduces the production of these inflammatory mediators.

Excretion
Primarily excreted via the urine (approximately 70%) and feces (approximately 20%) as metabolites.
Half life
Approximately 22 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations reached in approximately 1 hour.
Metabolism
Extensively metabolized, primarily via cytochrome P450 (CYP) enzymes, particularly CYP3A4, to inactive metabolites.

Contraindications
- Hypersensitivity to Etoricoxib or any excipients
- Active peptic ulceration or gastrointestinal bleeding
- Severe hepatic dysfunction
- Severe renal impairment (CrCl <30 mL/min)
- Congestive heart failure (NYHA Class II-IV)
- Established ischemic heart disease, peripheral arterial disease, and/or cerebrovascular disease
- Uncontrolled hypertension
Drug Interactions
Lithium
Increased plasma lithium levels.
Warfarin
Increased risk of bleeding, monitor INR.
Diuretics
Reduced diuretic and antihypertensive effects.
Methotrexate
Increased plasma methotrexate levels, potentially increasing toxicity.
Oral contraceptives
Increased plasma concentrations of ethinyl estradiol, potentially increasing side effects.
ACE inhibitors/Angiotensin II receptor antagonists
Reduced antihypertensive effect and increased risk of renal impairment.
