Description
Overview of the medicine
Losartan is used to treat high blood pressure, reduce the risk of stroke in patients with high blood pressure and an enlarged heart, and to treat kidney disease in patients with type 2 diabetes and high blood pressure.
Uses & Indications
Dosage
Adults
Initial dose 50 mg once daily, may be increased to 100 mg once daily or divided doses.
Elderly
No initial dosage adjustment generally needed, but monitor closely.
Renal_impairment
No initial dosage adjustment needed for mild to moderate impairment; consider lower starting dose for severe impairment or dialysis patients (25 mg once daily).
How to Take
Oral administration, with or without food.
Mechanism of Action
Blocks the binding of angiotensin II to the AT1 receptor, preventing vasoconstriction and aldosterone release.
Pharmacokinetics
Onset
6 hours for maximal effect.
Excretion
Excreted via urine (35%) and feces (60%) as unchanged drug and metabolites.
Half life
Losartan: 2 hours; active metabolite (E-3174): 6-9 hours.
Absorption
Well absorbed orally, undergoes extensive first-pass metabolism. Bioavailability is about 33%. Peak plasma concentrations within 1 hour for losartan, 3-4 hours for active metabolite.
Metabolism
Extensive hepatic metabolism (CYP2C9 and CYP3A4) to an active carboxylic acid metabolite (E-3174).
Side Effects
Contraindications
- Hypersensitivity to losartan or any component
- Pregnancy (second and third trimesters)
- Concomitant use with aliskiren in patients with diabetes or renal impairment
Drug Interactions
NSAIDs
May attenuate antihypertensive effect, increased risk of renal impairment.
Lithium
Increased serum lithium concentrations and toxicity.
Diuretics/other antihypertensives
Increased hypotensive effect.
Potassium-sparing diuretics/potassium supplements
Increased risk of hyperkalemia.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include hypotension and tachycardia; bradycardia may occur. Treatment is symptomatic and supportive. Hemodialysis is not effective.
Pregnancy & Lactation
Category D in 2nd and 3rd trimesters. Not recommended during breastfeeding due to potential harm to infant.
