Overview of the medicine
Creston 10 mg Tablet contains Rosuvastatin, which belongs to a group of medicines called statins. It is used to lower high cholesterol levels and reduce the risk of heart attack, stroke, and other cardiovascular problems.
Uses & Indications
Dosage
Adults
Initial dose is usually 5-10 mg once daily. Dosage may be adjusted at 2-4 week intervals. Maximum dose is 40 mg once daily.
Elderly
No dose adjustment is generally required, but a starting dose of 5 mg is recommended for patients >70 years old.
Renal_impairment
For severe renal impairment (CrCl <30 mL/min), start with 5 mg once daily and do not exceed 10 mg once daily.
How to Take
Take orally once daily, with or without food. It can be taken at any time of day, but it's often recommended to take it at the same time each day.
Mechanism of Action
Rosuvastatin works by inhibiting HMG-CoA reductase, an enzyme in the liver that plays a key role in the production of cholesterol. This leads to a reduction in LDL ('bad') cholesterol and triglycerides, and an increase in HDL ('good') cholesterol.
Pharmacokinetics
Onset
Significant lipid-lowering effects are seen within 1 week, with 90% of the maximum response achieved by 2 weeks and maximal effect achieved by 4 weeks.
Excretion
Approximately 90% is excreted unchanged in the feces, with the remainder excreted in the urine.
Half life
Approximately 19 hours.
Absorption
Peak plasma concentrations are achieved approximately 3 to 5 hours following oral administration. Absolute bioavailability is approximately 20%.
Metabolism
Limited metabolism (approximately 10%) mainly by CYP2C9 and to a lesser extent by CYP2A6.
Side Effects
Contraindications
- Hypersensitivity to rosuvastatin or any component of the formulation
- Active liver disease or unexplained persistent elevations of serum transaminases
- Severe renal impairment (CrCl <30 mL/min)
- Pregnancy and lactation
- Concomitant use of cyclosporine
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, requiring INR monitoring.
Gemfibrozil
Increases risk of myopathy/rhabdomyolysis; avoid concomitant use or limit rosuvastatin dose.
Cyclosporine
Concomitant use significantly increases rosuvastatin exposure; contraindicated.
Erythromycin
May decrease rosuvastatin exposure.
Antacids (Aluminum/Magnesium hydroxide)
Decreases rosuvastatin plasma concentrations; take antacids 2 hours after rosuvastatin.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
There is no specific antidote for rosuvastatin overdose. In the event of an overdose, treatment should be symptomatic and supportive measures should be instituted as required. Hemodialysis is unlikely to be of benefit.
Pregnancy & Lactation
Rosuvastatin is contraindicated during pregnancy and breastfeeding. Women of childbearing potential should use effective contraception during treatment.
