Creston 20 mg tablet contains Rosuvastatin, a statin medication used to lower high cholesterol and triglyceride levels, thereby reducing the risk of heart attack, stroke, and other cardiovascular problems. It works by blocking an enzyme in the liver responsible for making cholesterol.
Uses & Indications
Dosage
Adults
Initial dose 5-10 mg once daily, adjusted based on lipid levels, up to a maximum of 40 mg daily. For Creston 20 mg, it's often a maintenance dose.
Elderly
Starting dose of 5 mg once daily may be considered for patients >70 years.
Renal_impairment
For severe renal impairment (CrCl <30 mL/min/1.73m2), starting dose 5 mg once daily, maximum 10 mg once daily.
How to Take
Creston 20 mg tablet can be taken at any time of day, with or without food. It is recommended to take it at the same time each day to maintain consistent blood levels.
Mechanism of Action
Rosuvastatin selectively and competitively inhibits HMG-CoA reductase, the rate-limiting enzyme that converts 3-hydroxy-3-methylglutaryl coenzyme A to mevalonate, a precursor to cholesterol. This inhibition reduces cholesterol synthesis in the liver and increases the number of LDL receptors on the hepatocyte surface, enhancing uptake and catabolism of LDL.

Onset
Lipid-lowering effects typically observed within 1 week, maximum effect within 2-4 weeks.
Excretion
Primarily via feces (90%), with minimal renal excretion.
Half life
Approximately 19 hours.
Absorption
Oral absorption is about 20%. Peak plasma concentrations occur 3-5 hours after oral administration.
Metabolism
Primarily hepatic, limited metabolism via CYP2C9.
Side Effects
Contraindications

Drug Interactions
Gemfibrozil
Increased rosuvastatin exposure; avoid co-administration.
Cyclosporine
Increased rosuvastatin exposure (contraindicated).
Erythromycin
Decreased rosuvastatin exposure.
Niacin (lipid-lowering doses)
Increased risk of myopathy/rhabdomyolysis.
Warfarin/Coumarin anticoagulants
May potentiate anticoagulant effects; monitor INR.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Management involves symptomatic treatment and supportive measures. Monitor liver function and CK levels. Hemodialysis is unlikely to be of benefit.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation. Statins can cause fetal harm. Women of childbearing potential should use effective contraception.
