Description
Overview of the medicine
Crezyme 325 mg Tablet is a pancreatic enzyme supplement used to improve the digestion of food in patients who do not have enough pancreatic enzymes due to conditions like cystic fibrosis, chronic pancreatitis, or pancreatic cancer. It contains digestive enzymes—lipase, amylase, and protease—essential for breaking down fats, carbohydrates, and proteins.
Uses & Indications
Dosage
Adults
Dosage is highly individualized and depends on the degree of maldigestion and fat content of the diet. Typically, 1-2 tablets (325-650 mg) with each meal and 1 tablet with snacks. Doses should be adjusted to achieve an adequate response, usually starting with 500-2500 lipase units/kg/meal.
Elderly
No specific dosage adjustment is generally required for elderly patients, but individual response and tolerance should be monitored.
Renal_impairment
No specific dosage adjustment is necessary for patients with renal impairment, as pancreatin is not systemically absorbed.
How to Take
Crezyme 325 mg Tablet should be taken orally with meals or snacks. The tablets should be swallowed whole with plenty of fluid. Do not crush or chew the tablets, as this can inactivate the enzymes and cause irritation in the mouth.
Mechanism of Action
Crezyme 325 mg Tablet works by providing exogenous pancreatic enzymes (lipase, amylase, and protease) to supplement the insufficient endogenous enzymes in the gastrointestinal tract. These enzymes break down fats into fatty acids and glycerol, carbohydrates into simple sugars, and proteins into peptides and amino acids, facilitating their absorption.
Pharmacokinetics
Onset
Within minutes of ingestion with food.
Excretion
Degraded enzymes and inactive components are primarily excreted in the feces.
Half life
Pancreatin enzymes are quickly degraded within the gastrointestinal lumen, with no measurable systemic half-life for intact enzymes.
Absorption
Intact pancreatic enzymes are minimally absorbed from the gastrointestinal tract; their primary action is within the lumen. Some proteases may be degraded and absorbed as small peptides.
Metabolism
Enzymes are primarily degraded by other proteases within the GI tract.
Side Effects
Contraindications
- Known hypersensitivity to pancreatin, pork protein, or any component of the formulation.
- Acute pancreatitis (in the initial phase, until the inflammatory process subsides).
Drug Interactions
Antacids
May decrease the efficacy of pancreatin by altering gastric pH, leading to premature enzyme inactivation. Enteric-coated formulations are less affected.
Iron supplements
High doses of pancreatin may theoretically impair the absorption of iron, although clinical significance is usually minimal.
Acarbose/Miglitol
These alpha-glucosidase inhibitors may be less effective when taken with pancreatic enzymes, as pancreatin itself aids in carbohydrate breakdown.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
High doses of pancreatin, particularly in patients with cystic fibrosis, have been associated with fibrosing colonopathy. Symptoms of overdose may include nausea, abdominal discomfort, diarrhea, and hyperuricosuria. Treatment is symptomatic and supportive; reduction of dose is recommended.
Pregnancy & Lactation
Pancreatin is generally considered safe during pregnancy and lactation as it is not significantly absorbed systemically. However, it should only be used if clearly needed and under medical supervision. Consult your doctor.
