Description
Overview of the medicine
Cromolyn sodium ophthalmic solution is used to prevent and relieve symptoms of allergic conjunctivitis by stabilizing mast cells and inhibiting mediator release. It is typically used for conditions like allergic conjunctivitis, vernal keratoconjunctivitis, and giant papillary conjunctivitis.
Uses & Indications
Dosage
Adults
Instill 1-2 drops in each eye 4-6 times daily at regular intervals. Continued use is recommended for prophylactic effect.
Elderly
No specific dosage adjustment is required for elderly patients; same as adult dosage.
Renal_impairment
No specific dosage adjustment required due to minimal systemic absorption.
How to Take
For ophthalmic use only. Wash hands before and after use. Tilt head back, pull down lower eyelid to form a pocket, and instill drops into the conjunctival sac. Avoid touching the dropper tip to the eye or any other surface to prevent contamination.
Mechanism of Action
Cromolyn sodium is a mast cell stabilizer that prevents the release of inflammatory mediators (such as histamine, leukotrienes) from mast cells following exposure to allergens. It acts prophylactically by stabilizing the mast cell membrane, thereby inhibiting degranulation and the subsequent release of these inflammatory substances. It does not possess direct antihistaminic or anti-inflammatory properties.
Pharmacokinetics
Onset
Therapeutic effects may take several days to a few weeks of regular use to become apparent.
Excretion
Primarily excreted unchanged via urine and bile in approximately equal amounts.
Half life
Systemic half-life is short (approximately 80 minutes for parenteral administration), but local ophthalmic action is sustained.
Absorption
Poorly absorbed systemically from the eye. Minimal systemic absorption occurs.
Metabolism
Not significantly metabolized in humans.
Side Effects
Contraindications
- Hypersensitivity to cromolyn sodium or any component of the formulation.
Drug Interactions
General
No known significant drug interactions with ophthalmic cromolyn due to its minimal systemic absorption.
Storage
Store at room temperature (15-30°C or 59-86°F). Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Overdose is unlikely with ophthalmic use. If accidental ingestion occurs, symptomatic and supportive treatment should be initiated. Contact a poison control center if necessary.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. As it is unknown whether cromolyn sodium is excreted in human milk following ophthalmic administration, caution should be exercised when administered to a nursing mother.

Clinical Trials
Numerous clinical trials have demonstrated the efficacy and safety of cromolyn sodium for the treatment and prevention of allergic conjunctivitis, showing significant reduction in ocular itching, redness, and swelling compared to placebo.
Lab Monitoring
- Not generally required for ophthalmic use due to minimal systemic absorption.
Doctor Notes
- Emphasize the prophylactic nature of cromolyn sodium and the importance of regular, consistent application for optimal results, especially before anticipated allergen exposure.
- Advise patients on proper instillation technique to maximize drug delivery and minimize contamination risk.
- Inform patients that full therapeutic effect may not be evident for several days to weeks and to continue treatment as prescribed, even if symptoms improve.
- Remind patients to avoid contact lens wear during treatment or to follow recommended guidelines for reinsertion.
Patient Guidelines
- Do not wear soft contact lenses during treatment or if your eyes are red/irritated. Wait at least 15 minutes after instilling drops before reinserting contact lenses.
- This medicine is for prevention; regular and continuous use is crucial for the best therapeutic effect.
- Do not discontinue use abruptly, as symptoms may return.
- Avoid touching the dropper tip to any surface to prevent contamination of the solution.
- If you use other eye drops, wait at least 5 minutes between applications.
Missed Dose Advice
If a dose is missed, apply it as soon as possible. If it is almost time for the next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to make up for a missed one.
Driving Precautions
Cromolyn sodium eye drops may cause temporary blurred vision or stinging after instillation. Caution is advised when driving or operating machinery until vision clears and symptoms subside.
Lifestyle Advice
- Identify and avoid known allergens (e.g., pollen, pet dander, dust mites) that trigger your eye allergies.
- Wear sunglasses outdoors to protect your eyes from environmental allergens and irritants.
