Crotam-Plus 5-10 Lotion is a topical medication containing Crotamiton 10% w/v and Sulfur 5% w/v. It is primarily used for the treatment of scabies and relief of pruritus (itching) associated with various dermatological conditions.
Uses & Indications
Dosage
Adults
For scabies: Apply a thin layer to the entire body surface from the neck down, once daily for 3-5 days. Reapply after bathing. For pruritus: Apply to affected areas 2-3 times daily until itching subsides.
Elderly
No specific dosage adjustment required; use as per adult dosage.
Renal_impairment
No specific dosage adjustment required for topical use as systemic absorption is minimal.
How to Take
Wash and dry the skin thoroughly before application. For scabies, apply to the entire body from the neck down, paying attention to skin folds, between fingers and toes, and under nails. Avoid contact with eyes, mouth, and other mucous membranes. Repeat application as directed. For pruritus, apply to the affected area and rub in gently.
Mechanism of Action
Crotamiton acts as a scabicide and antipruritic. Its exact mechanism as a scabicide is unknown but thought to involve neurotoxic effects on the mites. As an antipruritic, it may exert a local anesthetic effect. Sulfur acts as a keratolytic, mild antiseptic, and weak antiparasitic agent. It is thought to exert its effect through conversion to hydrogen sulfide by epidermal cells, which is toxic to mites.

Onset
Antipruritic effect of Crotamiton may be observed within 30 minutes to an hour. Scabicidal action requires several days of application.
Excretion
Metabolites of Crotamiton are excreted via the kidneys. Sulfur is largely excreted via the skin and feces.
Half life
Not well established for topical Crotamiton or Sulfur due to limited systemic absorption.
Absorption
Crotamiton is absorbed percutaneously to a small extent (less than 10%) after topical application. Sulfur has minimal systemic absorption when applied topically.
