Description
Overview of the medicine
Cvnor-A 5 mg Tablet contains Amlodipine, a calcium channel blocker used to treat high blood pressure (hypertension) and angina (chest pain). It works by relaxing blood vessels, making it easier for the heart to pump blood.
Uses & Indications
Dosage
Adults
The usual initial dose for hypertension and angina is 5 mg orally once daily. The maximum dose is 10 mg once daily.
Elderly
A lower initial dose of 2.5 mg once daily may be considered.
Renal_impairment
No dosage adjustment is typically required for patients with renal impairment.
How to Take
Take Cvnor-A 5 mg Tablet orally with or without food, usually once daily. Swallow the tablet whole with water.
Mechanism of Action
Amlodipine inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. This direct action on vascular smooth muscle causes vasodilation and reduces peripheral vascular resistance, leading to a decrease in blood pressure.
Pharmacokinetics
Onset
Onset of antihypertensive effect is gradual, typically within 6-12 hours for peak effect.
Excretion
Primarily excreted in the urine as inactive metabolites (approximately 60%), with about 10% excreted unchanged.
Half life
Plasma elimination half-life ranges from 30-50 hours, allowing for once-daily dosing.
Absorption
Well absorbed from the gastrointestinal tract; absolute bioavailability is 64-90%. Peak plasma concentrations are reached 6-12 hours after oral dosing.
Metabolism
Extensively metabolized in the liver to inactive metabolites (approximately 90%).
Side Effects
Contraindications
- •Hypersensitivity to Amlodipine, other dihydropyridines, or any component of the formulation.
- •Severe hypotension, cardiogenic shock, and clinically significant aortic stenosis.
- •Unstable angina or myocardial infarction within the last month.
Drug Interactions
Sildenafil
May enhance the hypotensive effects of amlodipine. Administer with caution.
Simvastatin
Increased risk of myopathy/rhabdomyolysis; limit simvastatin dose to 20 mg daily in patients taking amlodipine.
CYP3A4 Inducers (e.g., Rifampin, St. John's Wort)
May decrease amlodipine exposure, potentially reducing its effectiveness. Monitor blood pressure closely.
CYP3A4 Inhibitors (e.g., Ketoconazole, Itraconazole, Ritonavir)
May increase amlodipine exposure, leading to increased hypotensive effects. Dosage adjustment may be necessary.
Storage
Store in a dry place below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include excessive peripheral vasodilation with marked hypotension and possibly reflex tachycardia. Management involves supportive measures, including cardiovascular monitoring. If severe hypotension occurs, administer intravenous fluids and vasopressors as needed.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Amlodipine is known to be excreted in human milk; caution should be exercised when administered to a nursing woman.
