Description
Overview of the medicine
Cytopag 25 mg Tablet is a synthetic prostaglandin E analogue used to prevent NSAID-induced gastric ulcers and for specific obstetric indications such as medical abortion and induction of labor.
Uses & Indications
Dosage
Adults
For NSAID-induced ulcer prevention: 25 mg once daily, preferably with food. For obstetric indications, dosage varies significantly and should be determined by a specialist.
Elderly
No specific dosage adjustment required, but use with caution due to potential increased sensitivity.
Renal_impairment
Initial dose reduction may be considered in severe renal impairment (CrCl < 30 mL/min).
How to Take
Take orally, preferably with food to minimize gastrointestinal side effects. Do not crush, chew, or split the tablet.
Mechanism of Action
Cytoprostil acts by decreasing gastric acid secretion, increasing bicarbonate and mucus production, and enhancing mucosal blood flow, thereby protecting the stomach lining. In the uterus, it causes contraction of the myometrial muscle fibers, leading to expulsion of uterine contents.
Pharmacokinetics
Onset
Gastric protection: 30 minutes; Uterine effects: 15-30 minutes.
Excretion
Primarily excreted in the urine (about 80%) and feces (about 15%) as inactive metabolites.
Half life
Approximately 20-40 minutes (active metabolite).
Absorption
Rapidly absorbed from the gastrointestinal tract; peak plasma concentrations reached within 30-60 minutes.
Metabolism
Extensively metabolized in the liver to the active free acid metabolite via de-esterification.
Side Effects
Contraindications
- Known hypersensitivity to Cytoprostil or other prostaglandins.
- Pregnancy (unless used for approved obstetric indications under strict medical supervision due to risk of abortion/birth defects).
- Active inflammatory bowel disease (Crohn's disease, ulcerative colitis) due to potential exacerbation of diarrhea.
Drug Interactions
Antacids (magnesium-containing)
May exacerbate diarrhea caused by Cytoprostil.
Oxytocin and other uterotonic agents
Increased risk of uterine hyperstimulation or rupture, especially during labor induction.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe diarrhea, abdominal pain, fever, lethargy, tremor, convulsions, palpitation, hypotension, and bradycardia. Management is supportive and symptomatic, focusing on fluid and electrolyte balance.
Pregnancy & Lactation
Highly contraindicated in pregnancy unless used for approved obstetric indications under strict medical supervision. If taken during pregnancy for other reasons, there is a significant risk of abortion, premature birth, or birth defects. Small amounts may pass into breast milk; use with caution in breastfeeding mothers.

Doctor Notes
- Counsel patients thoroughly, especially women of childbearing potential, about the abortifacient potential.
- Administer with extreme caution in patients with cardiovascular disease due to potential for hypotension.
- Monitor uterine activity closely if used for obstetric purposes.
