Description
Overview of the medicine
D-Coral DX is a combination supplement containing Calcium Carbonate and high-dose Vitamin D3 (Cholecalciferol), primarily used to treat and prevent calcium and vitamin D deficiencies, and support bone health. It is essential for strong bones, teeth, and proper functioning of nerves, muscles, and heart.
Uses & Indications
Dosage
Adults
Typically 1-2 tablets daily, or as directed by physician. Should be taken with meals.
Elderly
Same as adult dosage, but caution with renal impairment.
Renal_impairment
Dosage adjustment may be required; monitor serum calcium and phosphate levels. Avoid in severe renal impairment.
How to Take
Orally. Tablets can be swallowed whole or chewed. Best taken with meals to enhance absorption.
Mechanism of Action
Calcium is an essential mineral required for bone formation and maintenance, muscle contraction, nerve transmission, and blood clotting. Vitamin D3 facilitates the absorption of calcium from the gut, regulates calcium and phosphate levels in the body, and promotes bone mineralization.
Pharmacokinetics
Onset
Effects on calcium and vitamin D levels begin within hours, but clinical benefits for bone health may take weeks to months.
Excretion
Calcium is excreted primarily via feces (unabsorbed) and urine (excess absorbed). Vitamin D metabolites are excreted mainly in bile and feces, with some renal excretion.
Half life
Calcium: Varies. Vitamin D3: Initial half-life of 24 hours, but stored in adipose tissue with a slow release, leading to a biological half-life of several weeks.
Absorption
Calcium Carbonate is absorbed from the gastrointestinal tract, with absorption enhanced by Vitamin D. Vitamin D3 is absorbed from the small intestine, primarily in the jejunum and ileum, and transported to the liver and kidneys for activation.
Metabolism
Calcium is not metabolized. Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D (calcifediol) and then in the kidneys to 1,25-dihydroxyvitamin D (calcitriol), the active form.
Side Effects
Contraindications
- Hypersensitivity to any component
- Hypercalcaemia (high calcium levels in blood)
- Hypercalciuria (high calcium levels in urine)
- Nephrolithiasis (kidney stones)
- Severe renal impairment
Drug Interactions
Levothyroxine
Calcium can reduce absorption; take calcium several hours apart.
Bisphosphonates
Reduced absorption; take calcium at least 30 minutes after bisphosphonate.
Cardiac Glycosides
Risk of arrhythmias with hypercalcaemia.
Thiazide diuretics
May increase risk of hypercalcaemia.
Tetracyclines & Fluoroquinolones
Calcium can reduce absorption; take calcium at least 2-3 hours before or 4-6 hours after.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms: Nausea, vomiting, constipation, thirst, polyuria, fatigue, weakness, hypercalcaemia. Management: Discontinue treatment, rehydrate, monitor serum calcium. In severe cases, intravenous fluids, diuretics, calcitonin or corticosteroids may be used.
