Description
Overview of the medicine
D-Fozin M is a combination medication containing Desloratadine, a non-drowsy antihistamine, and Montelukast, a leukotriene receptor antagonist. It is used to treat symptoms of allergic rhinitis and asthma where both allergic and inflammatory components are present.
Uses & Indications
Dosage
Adults
One tablet (Desloratadine 5 mg + Montelukast 10 mg) once daily, preferably in the evening.
Elderly
No dose adjustment generally required, but caution with renal/hepatic impairment.
Renal_impairment
Caution and dosage adjustment may be needed based on individual drug components; consult a physician.
How to Take
Oral administration. May be taken with or without food. For asthma, it is usually taken in the evening.
Mechanism of Action
Desloratadine selectively antagonizes peripheral H1-receptors, providing relief from allergic symptoms. Montelukast selectively antagonizes the cysteinyl leukotriene 1 (CysLT1) receptor, inhibiting the actions of leukotrienes which are inflammatory mediators involved in asthma and allergic rhinitis.

Onset
Desloratadine: within 30 minutes for allergic symptoms. Montelukast: within 1 day for asthma.
Excretion
Desloratadine: equally in urine and feces as metabolites. Montelukast: primarily via bile in feces.
Half life
Desloratadine: approx. 27 hours; Montelukast: 2.7 to 5.5 hours.
Absorption
Desloratadine is rapidly absorbed, Tmax 3 hrs; Montelukast is rapidly absorbed, Tmax 2-4 hrs. Food does not significantly affect absorption for either.
Metabolism
Desloratadine: extensively metabolized to 3-hydroxydesloratadine by CYP2C8/3A4. Montelukast: extensively metabolized by CYP3A4, 2C9, 2C8.

Drug Interactions
Gemfibrozil
May increase plasma concentrations of montelukast.
Phenobarbital, Rifampin
May decrease plasma concentrations of montelukast.
Ketoconazole, Erythromycin, Azithromycin, Fluoxetine, Cimetidine
May increase plasma concentrations of desloratadine.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, tachycardia, headache, and abdominal pain. Treatment is symptomatic and supportive; no specific antidote is available. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category B. Should be used during pregnancy only if clearly needed. Excreted in breast milk, so caution is advised when administered to a nursing mother.
