Uses & Indications
Dosage
Adults
For severe Vitamin D deficiency: 20,000 IU once weekly for 7 to 10 weeks, followed by a maintenance dose as determined by serum 25(OH)D levels. For maintenance, typically 800-2000 IU daily or equivalent weekly/monthly dose.
Elderly
Similar to adults. May require higher doses due to reduced skin synthesis and dietary intake. Individual assessment is crucial.
Renal_impairment
Use with caution. Monitor serum calcium, phosphate, and 25(OH)D levels regularly. Dosage adjustment may be necessary. Not recommended in severe renal impairment without specialized advice.
How to Take
Take the capsule orally, preferably with a meal (especially fatty foods) to enhance absorption. Swallow whole with water, do not chew or crush.
Mechanism of Action
Cholecalciferol (Vitamin D3) is biologically inactive but is hydroxylated in the liver to 25-hydroxyvitamin D (calcifediol) and then in the kidneys to 1,25-dihydroxyvitamin D (calcitriol), the active form. Calcitriol promotes intestinal absorption of calcium and phosphate, regulates bone mineralization, and maintains calcium homeostasis.

Onset
Therapeutic effects may take several days to weeks to manifest, as Cholecalciferol requires metabolic activation and accumulation.
Excretion
Mainly excreted via the bile into the feces. A small amount is excreted renally.
Half life
The half-life of 25-hydroxyvitamin D (calcifediol) is about 15-20 days, while the half-life of 1,25-dihydroxyvitamin D (calcitriol) is 4-6 hours. Stored vitamin D in adipose tissue can be released over several weeks.
Absorption
Cholecalciferol is readily absorbed from the small intestine, primarily in the presence of bile salts and dietary fat.
Metabolism
Initially hydroxylated in the liver by cytochrome P450 enzymes (e.g., CYP2R1, CYP27A1) to 25-hydroxyvitamin D (calcifediol), then further hydroxylated in the kidneys by 1-alpha-hydroxylase (CYP27B1) to 1,25-dihydroxyvitamin D (calcitriol).

Drug Interactions
Corticosteroids

Cardiac glycosides (e.g., Digoxin)
Increased risk of toxicity due to potential hypercalcemia.
Orlistat, Cholestyramine, mineral oil
May impair absorption of Vitamin D.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose primarily relate to hypercalcemia and include nausea, vomiting, constipation, polyuria, polydipsia, weakness, headache, and confusion. Severe cases can lead to kidney damage, arrhythmias, and coma. Management involves discontinuation of Cholecalciferol, hydration, and medical supervision.
Pregnancy & Lactation
Pregnancy Category C. Adequate Vitamin D intake is crucial during pregnancy and lactation. However, high doses should be avoided. Consult your doctor to determine the appropriate dosage.
