Description
Overview of the medicine
Dalatic Plus is a topical gel containing Betamethasone Dipropionate, a potent corticosteroid, and Salicylic Acid, a keratolytic agent. It is used to treat various skin conditions characterized by hyperkeratosis and inflammation.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area once or twice daily. Do not use for more than 2 weeks continuously without medical advice.
Elderly
Same as adult dosage, but caution with systemic absorption due to thinner skin.
Renal_impairment
No specific dosage adjustment for topical use, but caution if extensive systemic absorption is expected due to salicylic acid.
How to Take
For external use only. Apply a thin film to the affected skin area and rub in gently. Wash hands before and after application. Avoid contact with eyes, face, genital, and anal areas, and mucous membranes.
Mechanism of Action
Betamethasone Dipropionate acts as a corticosteroid by inducing phospholipase A2 inhibitory proteins, thus reducing the release of arachidonic acid and subsequently inhibiting the formation of inflammatory mediators. Salicylic acid acts as a keratolytic agent by softening and desquamating the stratum corneum, allowing better penetration of the corticosteroid.
Pharmacokinetics
Onset
Topical effects usually within a few days.
Excretion
Betamethasone is excreted via urine. Salicylic acid and its metabolites are excreted primarily by the kidneys.
Half life
Systemic half-life of absorbed betamethasone is approximately 5 hours. Salicylic acid half-life depends on dose and formulation, typically 2-3 hours.
Absorption
Minimal systemic absorption, but can increase with prolonged use, extensive areas, or occlusive dressings. Salicylic acid can be absorbed systemically, especially from large areas.
Metabolism
Betamethasone is metabolized primarily in the liver. Salicylic acid is metabolized in the liver to salicyluric acid, salicyl phenolic glucuronide, and acyl glucuronide.
Side Effects
Contraindications
- Hypersensitivity to betamethasone, salicylic acid, or any component of the formulation
- Viral skin infections (e.g., herpes, varicella)
- Fungal skin infections (untreated)
- Bacterial skin infections (untreated)
- Perioral dermatitis
- Rosacea
- Acne vulgaris
- Children under 2 years of age
Drug Interactions
Other corticosteroids
Increased risk of systemic corticosteroid side effects.
Other topical salicylic acid preparations
Increased risk of salicylate toxicity.
Storage
Store below 25°C. Do not freeze. Keep out of reach of children.
Overdose
Topical overdose is unlikely to cause acute systemic effects. Chronic overdose or misuse may lead to symptoms of hypercorticism (Cushing's syndrome) or salicylate toxicity (e.g., tinnitus, dizziness). Treatment is symptomatic and supportive. Gradual withdrawal of corticosteroids may be necessary.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit outweighs the potential risk to the fetus. Avoid large areas, prolonged use, or occlusive dressings. Use with caution during lactation; advise mothers not to apply to the breast area.

Clinical Trials
Clinical trials have shown the efficacy and safety of Betamethasone Dipropionate 0.025% + Salicylic Acid 12% topical gel in treating hyperkeratotic and inflammatory dermatoses. Studies typically evaluate reduction in PASI score for psoriasis and overall lesion improvement.
Lab Monitoring
- No routine lab monitoring required for typical topical use. In cases of extensive use or suspected systemic absorption, HPA axis suppression (e.g., morning plasma cortisol levels) or salicylate levels might be monitored.
Doctor Notes
- Advise patients on proper application technique and duration of use.
- Monitor for signs of HPA axis suppression with extensive or prolonged use, especially in children.
- Counsel patients on potential local and systemic side effects.
Patient Guidelines
- Apply sparingly to the affected area.
- Do not use on open wounds or infected skin.
- Avoid prolonged use, especially on the face, groin, or axillae.
- Do not use with occlusive dressings unless advised by a doctor.
- Inform your doctor if symptoms do not improve within 2 weeks.
Missed Dose Advice
If a dose is missed, apply it as soon as you remember. If it's close to the time for the next dose, skip the missed dose and continue with your regular schedule. Do not apply a double dose.
Driving Precautions
Unlikely to affect ability to drive or operate machinery, as it is a topical preparation.
Lifestyle Advice
- Maintain good skin hygiene.
