Description
Overview of the medicine
Dapclin-5-Gel is a combination topical medication containing Adapalene and Clindamycin Phosphate, primarily used for the treatment of acne vulgaris. Adapalene is a retinoid-like compound that helps to normalize follicular keratinization and reduce inflammation, while Clindamycin is an antibiotic that inhibits the growth of acne-causing bacteria.
Uses & Indications
Dosage
Adults
Apply a thin layer of gel to the affected areas of the face and/or trunk once daily in the evening after washing.
Elderly
Dosage adjustment not typically required.
Renal_impairment
No dosage adjustment needed as systemic absorption is minimal.
How to Take
For topical use only. Clean the affected skin thoroughly before application. Apply a thin layer to the entire affected area (not just individual pimples) once daily in the evening. Avoid contact with eyes, lips, nostrils, and mucous membranes. Wash hands after use.
Mechanism of Action
Adapalene acts as a retinoid receptor modulator, normalizing follicular differentiation, reducing microcomedone formation, and exhibiting anti-inflammatory properties. Clindamycin is a lincosamide antibiotic that binds to the 50S ribosomal subunit of susceptible bacteria, inhibiting protein synthesis and bacterial growth. In acne, it targets Propionibacterium acnes.
Pharmacokinetics
Onset
Clinical improvement typically seen within 1-4 weeks, with optimal results after 8-12 weeks.
Excretion
Primarily excreted via biliary and renal routes for systemically absorbed drug; most is locally degraded or remains on skin.
Half life
Not well-defined for topical use due to minimal systemic absorption.
Absorption
Minimal systemic absorption after topical application. Adapalene: <0.25% absorbed. Clindamycin: <5% absorbed.
Metabolism
Adapalene is primarily metabolized by O-demethylation, hydroxylation, and conjugation. Clindamycin is metabolized in the liver.
Side Effects
Contraindications
- •Hypersensitivity to adapalene, clindamycin, or any component of the gel
- •History of regional enteritis, ulcerative colitis, or antibiotic-associated colitis
Drug Interactions
Neuromuscular blocking agents
Systemic clindamycin has neuromuscular blocking properties; caution with concomitant use, though minimal with topical.
Other topical acne therapies (e.g., benzoyl peroxide, salicylic acid)
Concomitant use may increase irritation. If used together, apply at different times of the day (e.g., one in morning, one in evening).
Storage
Store below 30°C in a dry place, away from direct light and moisture. Do not freeze.
Overdose
Excessive application does not lead to faster or better results and may result in marked redness, peeling, or discomfort. Management includes symptomatic and supportive measures. In case of accidental ingestion, seek medical attention.
Pregnancy & Lactation
Adapalene is Pregnancy Category C. Clindamycin is Pregnancy Category B. Avoid use during pregnancy unless the potential benefit justifies the potential risk to the fetus. Use with caution during lactation; consult a healthcare professional.
