Overview of the medicine
Daprostat is a brand of Dapoxetine, a selective serotonin reuptake inhibitor (SSRI) used for the treatment of premature ejaculation (PE) in adult men aged 18 to 64 years. It helps to prolong the time until ejaculation and improve control over ejaculation.
Uses & Indications
Dosage
Adults
Initial dose of 30 mg, taken orally approximately 1 to 3 hours before anticipated sexual activity. If the effect of 30 mg is insufficient and side effects are acceptable, the dose may be increased to a maximum of 60 mg. It should not be taken more than once every 24 hours.
Elderly
Efficacy and safety have not been established in patients aged 65 years and over. Not recommended.
Renal_impairment
Caution is advised in patients with mild or moderate renal impairment. Not recommended in severe renal impairment.
How to Take
Take orally with a full glass of water, with or without food. It should be taken approximately 1 to 3 hours before anticipated sexual activity. Do not take more than one dose in 24 hours.
Mechanism of Action
Dapoxetine is a potent and selective serotonin reuptake inhibitor (SSRI). Its therapeutic effect in premature ejaculation is presumed to be related to the inhibition of neuronal reuptake of serotonin and the subsequent potentiation of serotonin's effect at pre- and post-synaptic receptors. It delays the ejaculatory reflex by affecting areas in the central nervous system.

Half life
Terminal half-life is approximately 1.5-2 hours for dapoxetine and 18-24 hours for its active metabolite desmethyldapoxetine.

Rapidly absorbed, Cmax reached 1-2 hours after oral administration.
Metabolism
Extensively metabolized in the liver by CYP2D6, CYP3A4, and flavin monooxygenase 1 (FMO1).
Side Effects
Contraindications
- Known hypersensitivity to dapoxetine or any excipients.
- Significant pathological cardiac conditions (e.g., heart failure, conduction abnormalities, ischemic heart disease).
- Moderate to severe hepatic impairment.
- Concomitant treatment with monoamine oxidase inhibitors (MAOIs) or within 14 days of discontinuing MAOI treatment.
- Concomitant treatment with thioridazine or within 14 days of discontinuing thioridazine.
- Concomitant treatment with other SSRIs, SNRIs, tricyclic antidepressants, or other serotonergic products.
- Concomitant use with potent CYP3A4 inhibitors (e.g., ketoconazole, ritonavir, telithromycin, nefazodone, nelfinavir, saquinavir).
- History of mania or severe depression.
Drug Interactions
MAOIs
Serious, sometimes fatal reactions including hyperthermia, rigidity, myoclonus, autonomic instability, mental status changes (including extreme agitation progressing to delirium and coma).
Alcohol
Increased risk of syncope and other CNS depressive effects.
Thioridazine
Increased plasma levels of thioridazine, leading to QT prolongation and serious ventricular arrhythmias.
SSRI/SNRI/TCA/Lithium
Increased risk of serotonin syndrome.
Potent CYP3A4 inhibitors (e.g., ketoconazole, ritonavir)
Increased dapoxetine exposure, risk of adverse effects.
Moderate CYP3A4 inhibitors (e.g., erythromycin, diltiazem, verapamil)
Increased dapoxetine exposure, caution advised.
Storage
Store below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, gastrointestinal disturbances (nausea, vomiting), tachycardia, tremor, and dizziness. Management is supportive and symptomatic.
Pregnancy & Lactation
Dapoxetine is not indicated for use in women. There are no adequate data from the use of dapoxetine in pregnant or lactating women.
