Overview of the medicine
Dapagliflozin is an oral antidiabetic drug used to improve glycemic control in adults with type 2 diabetes mellitus, and for the treatment of heart failure with reduced ejection fraction and chronic kidney disease.
Uses & Indications
Dosage
Adults
For Type 2 Diabetes Mellitus: Initial dose is typically 5 mg once daily, may be increased to 10 mg once daily. For Heart Failure or CKD: 10 mg once daily. The 1 mg tablet may be used for specific titration or in very sensitive patients, though 5 mg is the usual starting dose.
Elderly
No dosage adjustment needed based on age, but consider renal function and risk of volume depletion.
Renal_impairment
Dose adjustment required based on eGFR. Not recommended if eGFR is below 25 mL/min/1.73m² for glucose lowering. For Heart Failure/CKD, initiation is not recommended if eGFR <25 mL/min/1.73m².
How to Take
Take orally, once daily, with or without food.
Mechanism of Action
It inhibits Sodium-Glucose Co-transporter 2 (SGLT2) in the renal tubules, leading to reduced glucose reabsorption and increased urinary glucose excretion, thereby lowering blood glucose levels.

Onset
Glucose lowering effect observed within hours of administration.
Excretion
Approximately 75% renally excreted and 21% fecally excreted, primarily as metabolites.
Half life
Approximately 12.9 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations reached within 1 hour. High oral bioavailability.
Metabolism
Primarily metabolized by UGT1A9 to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to dapagliflozin or any excipients.
- Patients with severe renal impairment, end-stage renal disease (ESRD), or on dialysis.
Drug Interactions
Lithium
Dapagliflozin may decrease lithium concentrations. Monitor lithium levels.
Diuretics
May increase the risk of dehydration and hypotension.
Insulin and Insulin Secretagogues (e.g., sulfonylureas)
May increase the risk of hypoglycemia. A lower dose of insulin or sulfonylurea may be required.
Storage
Store below 30°C in a dry place, protected from light and moisture.
Overdose
In the event of an overdose, contact a poison control center or emergency room immediately. Symptomatic and supportive treatment should be initiated.
Pregnancy & Lactation
Not recommended during the second and third trimesters of pregnancy due to potential for adverse renal development in the fetus. Not recommended during breastfeeding as it may pass into breast milk.
