Description
Overview of the medicine
Darbepoetin alfa is an erythropoiesis-stimulating protein closely related to erythropoietin. It is used to treat anemia associated with chronic kidney disease and chemotherapy-induced anemia.
Uses & Indications
Dosage
Adults
Initial 0.45 mcg/kg SC or IV once weekly, or 0.75 mcg/kg SC every two weeks. Doses adjusted to maintain Hb levels between 10-12 g/dL.
Elderly
No specific dosage adjustment, but careful monitoring.
Renal_impairment
Used to treat anemia in CKD patients. Dosage is adjusted based on hemoglobin levels.
How to Take
Subcutaneous (SC) or Intravenous (IV) injection. Administered by a healthcare professional. Do not shake.
Mechanism of Action
Darbepoetin alfa stimulates erythropoiesis by binding to erythropoietin receptors on progenitor cells in the bone marrow, leading to red blood cell production.
Pharmacokinetics
Onset
Hemoglobin levels typically begin to rise within 2-6 weeks.
Excretion
Primarily via catabolism. Minimal renal excretion of intact drug.
Half life
Approximately 25.3 hours (IV) to 48.8 hours (SC) in patients with chronic kidney disease.
Absorption
Subcutaneous administration leads to slower, prolonged absorption. IV administration is immediate. Bioavailability SC is 37-84%.
Metabolism
Catabolized like endogenous proteins.
Side Effects
Contraindications
- Uncontrolled hypertension.
- Pure red cell aplasia (PRCA) that begins after treatment with darbepoetin alfa or other erythropoietin protein drugs.
- Serious allergic reactions to darbepoetin alfa.
Drug Interactions
No significant drug-drug interactions
No significant drug-drug interactions have been identified with Darbepoetin Alfa.
Storage
Store refrigerated at 2°C to 8°C (36°F to 46°F). Do not freeze. Protect from light.
Overdose

Pregnancy & Lactation
Use during pregnancy only if potential benefit justifies potential risk to the fetus. Caution when administering to a nursing mother.
