Description
Overview of the medicine
Daxetin 30 mg tablet contains Dapoxetine, a selective serotonin reuptake inhibitor (SSRI) used for the treatment of premature ejaculation in adult men aged 18 to 64 years.
Uses & Indications
Dosage
Adults
Initial dose is 30 mg, taken orally as needed approximately 1 to 3 hours prior to anticipated sexual activity. Do not take more than one dose in 24 hours. If 30 mg is insufficient and side effects are acceptable, the dose may be increased to 60 mg.
Elderly
Not recommended for patients 65 years or older due to limited data.
Renal_impairment
Caution is advised in patients with mild to moderate renal impairment. Not recommended in severe renal impairment.
How to Take
Taken orally with a full glass of water, with or without food, approximately 1 to 3 hours prior to anticipated sexual activity.
Mechanism of Action
Dapoxetine inhibits the serotonin transporter, increasing serotonin's action at the postsynaptic cleft and thereby inhibiting the ejaculatory reflex, leading to delayed ejaculation.
Pharmacokinetics
Onset
Effect typically observed within 1-3 hours after administration.
Excretion
Mainly via urine, as metabolites.
Half life
Terminal half-life is approximately 1.5-2 hours, with an active metabolite having a longer half-life.
Absorption
Rapidly absorbed. Peak plasma concentration is reached within 1-2 hours.
Metabolism
Primarily hepatic, via CYP2D6 and CYP3A4, to various active and inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to dapoxetine or any excipients.
- Significant pathological cardiac conditions (e.g., heart failure, conduction abnormalities like AV block, sick sinus syndrome).
- Concomitant use with monoamine oxidase inhibitors (MAOIs) or within 14 days of discontinuing MAOIs.
- Concomitant use with thioridazine or within 14 days of discontinuing thioridazine.
- Concomitant use with serotonin reuptake inhibitors (SSRIs, SNRIs, TCAs) or within 14 days of discontinuing these agents.
- Moderate to severe hepatic impairment.
- Concomitant use with potent CYP3A4 inhibitors (e.g., ketoconazole, ritonavir, telithromycin, nefazodone, nelfinavir, saquinavir).
Drug Interactions
Alcohol
Exacerbates neurocognitive effects (e.g., dizziness, somnolence) and may increase syncope risk. Avoid.
Thioridazine
Increased QT prolongation and ventricular arrhythmias; contraindicated.
SSRIs/SNRIs/TCAs
Increased risk of serotonin syndrome; contraindicated.
Monoamine Oxidase Inhibitors (MAOIs)
Increased risk of serotonin syndrome; contraindicated.
Potent CYP3A4 Inhibitors (e.g., Ketoconazole, Ritonavir)
Significantly increased dapoxetine exposure; contraindicated.
Moderate CYP3A4 Inhibitors (e.g., Erythromycin, Verapamil, Diltiazem)
Increased dapoxetine exposure; caution, consider lower starting dose.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dizziness, somnolence, tremor, nausea, and vomiting. Treatment is symptomatic and supportive; there is no specific antidote.
Pregnancy & Lactation
