Overview of the medicine
DDR 60 mg capsule contains Dexlansoprazole, a proton pump inhibitor (PPI) used to treat acid-related disorders of the stomach and esophagus, such as gastroesophageal reflux disease (GERD) and erosive esophagitis.
Uses & Indications
Dosage
Adults
Healing of EE: 60 mg once daily for up to 8 weeks. Maintenance of EE: 30 mg or 60 mg once daily for up to 6 months. Symptomatic GERD: 30 mg once daily for 4 weeks.
Elderly
No dosage adjustment necessary based on age.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment. No data for severe impairment.
How to Take
Take orally once daily, with or without food. Swallow the capsule whole. Do not chew or crush. For patients who have difficulty swallowing, the capsule can be opened and the granules mixed with applesauce.
Mechanism of Action
Dexlansoprazole inhibits the H+/K+-ATPase enzyme system (the proton pump) at the secretory surface of the gastric parietal cell. This leads to a dose-dependent inhibition of both basal and stimulated acid secretion.

Onset
Within hours, full effect within a few days.
Excretion
Primarily excreted in urine (approx. 51%) and feces (approx. 47%) as metabolites.
Half life
Approximately 1-2 hours.
Absorption
Well absorbed after oral administration. Peak plasma concentration achieved in 1-2 hours (first release) and 4-5 hours (second release).
Metabolism
Extensively metabolized in the liver by cytochrome P450 (CYP2C19 and CYP3A4).

Contraindications
- Hypersensitivity to dexlansoprazole or any component of the formulation
- Concurrent use with rilpivirine-containing products
Drug Interactions
Warfarin
Monitor INR; potential for increased bleeding risk.
Tacrolimus
Increased systemic exposure of tacrolimus has been reported; monitor tacrolimus concentrations.
Clopidogrel
Potential for reduced efficacy of clopidogrel; consider alternative antiplatelet therapy or PPI.
Methotrexate
May increase methotrexate levels, especially in high-dose therapy; consider temporary PPI withdrawal.
Drugs dependent on gastric pH for absorption (e.g., ketoconazole, ampicillin esters, iron salts)
May reduce absorption.
Storage
Store at room temperature (below 30°C), away from moisture and light. Keep out of reach of children.
Overdose
Symptoms of overdose are generally minimal and similar to known adverse events. Management is symptomatic and supportive. Dexlansoprazole is not significantly removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Caution advised during breastfeeding; it is unknown if dexlansoprazole is excreted in human milk.
