Description
Overview of the medicine
Debocal 500 mg tablet is a calcium supplement used to prevent or treat low blood calcium levels in people who do not get enough calcium from their diets. It is crucial for bone formation, muscle contraction, and nerve function.
Uses & Indications
Dosage
Adults
For calcium supplementation: 500-1000 mg daily, usually in divided doses with meals. As an antacid: 500-1500 mg as needed. Do not exceed 2500 mg/day of elemental calcium.
Elderly
Similar to adults, but careful monitoring may be needed, especially with concomitant conditions or medications.
Renal_impairment
Use with caution and monitor serum calcium and phosphate levels closely. May be used as a phosphate binder in chronic renal failure under medical supervision.
How to Take
Take orally, preferably with meals, to enhance absorption. If used as an antacid, it can be taken with or without food. Swallow tablets whole with water.
Mechanism of Action
Calcium Carbonate provides elemental calcium, an essential mineral for numerous physiological processes. It maintains normal bone structure and function, helps in blood clotting, muscle contraction, and nerve transmission. As an antacid, it neutralizes stomach acid.
Pharmacokinetics
Onset
Acute effects (e.g., antacid) are rapid. For bone health, effects are observed over weeks to months.
Excretion
Primarily via the kidneys (urine) and feces (unabsorbed calcium).
Half life
Calcium homeostasis is tightly regulated; there isn't a single plasma half-life for elemental calcium.
Absorption
Variable, approximately 20-30% absorbed from the gastrointestinal tract, optimized with food and vitamin D. Absorption can be affected by gastric pH.
Metabolism
Not metabolized in the conventional sense; elemental calcium is utilized or excreted.
Side Effects
Contraindications
- •Hypercalcemia (high blood calcium levels)
- •Severe hypercalciuria (high calcium in urine)
- •Severe renal impairment (unless used as a phosphate binder under strict supervision)
- •Known hypersensitivity to calcium carbonate or any excipients
Drug Interactions
Bisphosphonates
Calcium can reduce the absorption. Take calcium at least 30 minutes (for some bisphosphonates) to 2 hours (for others) after bisphosphonate.
Iron supplements
Calcium may interfere with iron absorption. Take at different times.
Thiazide diuretics
Can decrease urinary calcium excretion, potentially leading to hypercalcemia. Monitor calcium levels.
Calcium channel blockers
Potential for reduced effectiveness of calcium channel blockers if calcium levels are elevated.
Thyroid hormones (e.g., Levothyroxine)
Reduced absorption of thyroid hormone. Administer calcium at least 4 hours apart.
Tetracycline and Fluoroquinolone antibiotics
Calcium can reduce the absorption of these antibiotics. Take calcium at least 2-4 hours apart.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose (hypercalcemia) may include nausea, vomiting, constipation, abdominal pain, muscle weakness, confusion, fatigue, excessive thirst, and increased urination. Severe cases can lead to kidney stones, cardiac arrhythmias, and coma. Treatment involves discontinuation of calcium, hydration, and medical management of hypercalcemia.
Pregnancy & Lactation
Calcium is essential during pregnancy and lactation. Debocal 500 mg tablet can be used during pregnancy and breastfeeding if medically indicated, ensuring the recommended daily intake is not exceeded. Consult a healthcare professional.
