Description
Overview of the medicine
Decomit 100 mcg Inhaler contains Fluticasone Propionate, a corticosteroid used to prevent and control asthma symptoms by reducing inflammation in the airways.
Uses & Indications
Dosage
Adults
Initially 100-250 mcg twice daily. May be adjusted based on response up to a maximum of 1000 mcg twice daily for severe cases.
Elderly
Similar to adult dosage; no specific dosage adjustment is usually required based on age, but clinical response should be monitored.
Renal_impairment
No dosage adjustment required in patients with renal impairment.
How to Take
For oral inhalation only. Shake the inhaler well before each use. Rinse your mouth with water and spit it out after each use to prevent oral candidiasis.
Mechanism of Action
Fluticasone Propionate works by binding to glucocorticoid receptors, leading to a reduction in inflammation and hyperreactivity in the airways. It suppresses the immune response and reduces the production of inflammatory mediators.
Pharmacokinetics
Onset
Onset of action within 24 hours, with maximal therapeutic effect achieved within 1-2 weeks of regular use.
Excretion
Mainly in feces as metabolites; approximately 1-5% excreted in urine.
Half life
Approximately 7.8 hours (terminal half-life after IV administration).
Absorption
Low systemic bioavailability (approximately 10-30% after inhalation) due to extensive first-pass metabolism and incomplete absorption from the gastrointestinal tract.
Metabolism
Primarily hepatic, via the cytochrome P450 3A4 (CYP3A4) isoenzyme.
Side Effects
Contraindications
- Hypersensitivity to fluticasone propionate or any excipients in the formulation.
- Primary treatment of status asthmaticus or other acute episodes of asthma where intensive measures are required.
Drug Interactions
Ritonavir
Concomitant use with potent CYP3A4 inhibitors like ritonavir is not recommended as it can significantly increase systemic exposure to fluticasone, leading to potential corticosteroid side effects.
Ketoconazole
Other moderate CYP3A4 inhibitors like ketoconazole may also increase systemic exposure to fluticasone, requiring caution.
Storage
Store below 30°C (86°F). Protect from direct sunlight and freezing. Keep out of reach of children. Do not puncture or incinerate the canister.
Overdose
Acute overdose is unlikely to cause acute problems. Chronic overdose may lead to systemic corticosteroid effects, including adrenal suppression. Management involves symptomatic and supportive treatment, and gradual withdrawal if necessary.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Consult a doctor. Caution should be exercised when administered to a nursing mother.

WHO Essential Medicine
YesClinical Trials
Extensive clinical trials have demonstrated the efficacy and safety of fluticasone propionate inhalers in the long-term management of asthma, showing significant improvements in lung function and reduction in asthma exacerbations.
Lab Monitoring
- Monitor for signs of adrenal suppression (e.g., morning cortisol levels) during long-term, high-dose therapy.
- Periodic eye examinations for patients on long-term treatment to check for cataracts or glaucoma.
Doctor Notes
- Educate patients thoroughly on proper inhaler technique and the importance of rinsing the mouth after use.
- Emphasize that this medication is a controller and not for acute asthma attacks; patients should carry a rescue inhaler.
- Monitor for systemic corticosteroid side effects, especially in patients on high doses or those switching from oral corticosteroids.
Patient Guidelines
- Use the inhaler regularly as prescribed, even when you feel well, for optimal asthma control.
- Do not use this inhaler for sudden breathing problems; use your rescue inhaler instead.
- Always rinse your mouth with water after each use to help prevent oral fungal infections (thrush).
- Learn and practice proper inhaler technique as demonstrated by your doctor or pharmacist.
Missed Dose Advice
If you miss a dose, take it as soon as you remember, unless it is almost time for your next dose. Do not take a double dose to make up for the missed one.
Driving Precautions
Fluticasone Propionate inhaler is generally not expected to affect the ability to drive or operate machinery. However, if you experience dizziness or visual disturbances, avoid driving.
Lifestyle Advice
- Identify and avoid triggers that worsen your asthma symptoms (e.g., allergens, smoke, pollution).
- Maintain a healthy lifestyle, including regular exercise and a balanced diet, as advised by your healthcare provider.
