Overview of the medicine
Deferox is an oral iron chelating agent used for the treatment of chronic iron overload, particularly in patients with transfusion-dependent anemias.
Uses & Indications
Dosage
Adults
Typically 10-30 mg/kg orally once daily, adjusted based on iron burden.
Elderly
Start with lower doses and monitor closely for adverse effects.
Renal_impairment
Dose reduction required; contraindicated in severe renal impairment.
How to Take
Take orally once daily, preferably with a light meal at the same time each day.
Mechanism of Action
Deferox binds to ferric iron (Fe3+) in a 2:1 ratio to form a stable complex, which is then excreted, primarily via bile. This reduces systemic iron levels.

Onset
Iron chelation activity begins within hours.
Excretion
Mainly through bile/feces and to a lesser extent via urine.
Half life
Approximately 5-10 hours.
Absorption
Rapidly absorbed after oral administration; bioavailability is moderate.
Metabolism
Extensively metabolized, primarily by glucuronidation.
Side Effects
Contraindications
- •Known hypersensitivity to Deferox or any excipients.
- •Severe renal impairment (creatinine clearance < 10 mL/min).
- •Myelodysplastic syndromes with low bone marrow iron.
Drug Interactions
Vitamin C
Co-administration with high doses of Vitamin C may increase iron toxicity, especially in patients with severe cardiac iron overload. Avoid doses >200 mg/day of Vitamin C.
Antacids (aluminum-containing)
May reduce Deferox absorption. Take at least 2 hours apart.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include gastrointestinal disturbances, headache, dizziness, and visual/auditory disturbances. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit outweighs the potential risk to the fetus. Not recommended during breastfeeding due to lack of data on excretion into breast milk.
