Description
Overview of the medicine
D-Fill (Deflazacort) is a corticosteroid used to treat a wide range of inflammatory and autoimmune conditions, including Duchenne muscular dystrophy, asthma, and rheumatic disorders. It has anti-inflammatory and immunosuppressive properties.
Uses & Indications
Dosage
Adults
Initial dosage varies based on condition, typically 6-18 mg once daily. Maintenance dose is individualized.
Elderly
Generally, the lowest effective dose should be used, with caution due to increased risk of side effects.
Renal_impairment
No specific dose adjustment is usually required, but caution is advised in severe cases.
How to Take
Take orally, preferably with food to minimize gastrointestinal upset. Do not chew or crush the tablet.
Mechanism of Action
Deflazacort acts as a glucocorticoid receptor agonist, inhibiting inflammatory mediators and suppressing immune responses. It also stabilizes lysosomal membranes and prevents the release of destructive enzymes.
Pharmacokinetics
Onset
Variable, typically within hours for anti-inflammatory effects.
Excretion
Excreted primarily in urine (approximately 70%) and feces (approximately 30%) as metabolites.
Half life
Active metabolite (21-desacetyl deflazacort) has a plasma half-life of 1.5-2 hours.
Absorption
Well absorbed orally. Peak plasma concentrations are achieved within 1.5-2 hours.
Metabolism
Rapidly converted to the active metabolite 21-desacetyl deflazacort, primarily by esterases.
Side Effects
Contraindications
- Hypersensitivity to Deflazacort or any component
- Systemic fungal infections
- Administration of live attenuated vaccines in patients receiving immunosuppressive doses
Drug Interactions
NSAIDs
Increased risk of gastrointestinal bleeding/ulceration.
Antacids
May reduce absorption of deflazacort; administer at least 2 hours apart.
Oral Contraceptives
May increase deflazacort levels and effects.
Diuretics (Thiazide/Loop)
Increased risk of hypokalemia.
Oral Hypoglycemics/Insulin
May reduce hypoglycemic effect; dose adjustment may be necessary.
Phenytoin, Phenobarbital, Rifampin
May increase deflazacort clearance, reducing its effect.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is rare. Symptoms may include exaggerated side effects like fluid retention, moon face, or high blood pressure. Treatment is symptomatic and supportive. No specific antidote.
