Description
Overview of the medicine
Demobet Cream contains Clobetasol Propionate, a highly potent topical corticosteroid used to reduce inflammation and itching associated with skin conditions like eczema, psoriasis, and dermatitis.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin area once or twice daily for up to 2 weeks. Do not use for more than 2 weeks without re-evaluation.
Elderly
Use with caution. Dosage adjustments may be necessary due to potential skin fragility and increased systemic absorption.
Renal_impairment
No specific dosage adjustments required for topical use, but systemic absorption could be a concern with severe impairment.
How to Take
For topical use only. Apply a thin layer to the affected area and gently rub in. Wash hands before and after application.
Mechanism of Action
Clobetasol Propionate acts by inducing phospholipase A2 inhibitory proteins, lipocortins. These proteins control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor, arachidonic acid.
Pharmacokinetics
Onset
Rapid onset of anti-inflammatory effects, usually within hours to days.
Excretion
Metabolites excreted primarily in urine, some in bile.
Half life
Not specifically well-defined for topical use, but systemic half-life of corticosteroids generally range from 18-36 hours.
Absorption
Significant absorption can occur through the skin, especially with prolonged use, occlusive dressings, or on compromised skin. Systemic absorption varies based on area and integrity of skin.
Metabolism
Primarily metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to Clobetasol Propionate or any components of the formulation.
- Rosacea, acne vulgaris, perioral dermatitis, pruritus without inflammation.
- Bacterial, fungal, viral, or parasitic infections of the skin (without concomitant anti-infective therapy).
Drug Interactions
CYP3A4 inhibitors (e.g., Ritonavir, Itraconazole)
Systemic exposure to clobetasol may be increased, potentially leading to systemic corticosteroid effects. Use with caution.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze.
Overdose
Acute overdose is unlikely. Chronic overuse or misuse can lead to systemic corticosteroid effects like Cushing's syndrome, hyperglycemia, or HPA axis suppression.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Use with caution during lactation; avoid application on breast area.
