Overview of the medicine
Depanil 2 mg Tablet contains Flupenthixol, a thioxanthene derivative antipsychotic. It is used in the treatment of mild to moderate anxiety, depression, and asthenia, often in low doses.
Uses & Indications
Dosage
Adults
Initially 0.5 mg to 1 mg orally once daily, increased to 2 mg orally once daily if needed. Maximum 3 mg daily for depression/anxiety.
Elderly
Lower doses should be used, e.g., 0.5 mg orally once daily. Titrate carefully.
Renal_impairment
No specific dose adjustment is usually required, but caution is advised in severe cases.
How to Take
Take orally with water, usually once daily in the morning to avoid insomnia. Can be taken with or without food.
Mechanism of Action
Flupenthixol acts as a dopamine receptor antagonist, primarily blocking D1 and D2 receptors. At low doses, it also has antidepressant and anxiolytic effects, possibly due to its effect on noradrenergic and serotonergic systems.

Onset
Anxiolytic/antidepressant effects may be seen within days to weeks. Antipsychotic effects might take longer.
Excretion
Mainly via feces and urine.

Approximately 35 hours (oral).
Absorption
Well absorbed orally, with peak plasma concentrations reached in 3-8 hours.
Metabolism
Extensively metabolized in the liver, primarily via sulfoxidation, N-dealkylation, and glucuronidation.

Drug Interactions
Anticholinergics
Increased anticholinergic effects (e.g., dry mouth, constipation).
Antihypertensives
Potentiated hypotensive effect.
QT-prolonging drugs
Increased risk of QT prolongation and arrhythmias.
Dopaminergic agents (e.g., levodopa)
Reduced effect of dopaminergic agents.
CNS depressants (alcohol, barbiturates, opioids)
Increased sedation and respiratory depression.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms include somnolence, coma, extrapyramidal symptoms, hypotension, shock, hyperthermia/hypothermia. Treatment is symptomatic and supportive, maintain airway, monitor cardiovascular and respiratory functions. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the potential benefits outweigh the potential risks. Neonates exposed to antipsychotic drugs during the third trimester are at risk for extrapyramidal and/or withdrawal symptoms. Excreted in breast milk, caution advised. Consult a doctor.
