Description
Overview of the medicine
Depo-Pred 40 mg Injection is a corticosteroid medication containing methylprednisolone acetate. It is used to treat various inflammatory and autoimmune conditions by reducing inflammation and suppressing the immune system.
Uses & Indications
Dosage
Adults
Intramuscular (IM) injection: 40-120 mg weekly or every two weeks, depending on condition. Intra-articular/soft tissue injection: 4-80 mg depending on joint size and severity.
Elderly
No specific dose adjustments required, but lower initial doses may be considered due to increased susceptibility to side effects and co-morbidities.
Renal_impairment
No dose adjustment generally required, as renal excretion of active drug is minimal.
How to Take
Administered by deep intramuscular (IM) injection into the gluteal muscle or intra-articularly/soft tissue by a healthcare professional. Not for intravenous (IV) use due to suspension formulation.
Mechanism of Action
Methylprednisolone is a glucocorticoid that binds to intracellular glucocorticoid receptors, modulating gene expression. This leads to reduced synthesis of inflammatory mediators (prostaglandins, leukotrienes), decreased migration of leukocytes to sites of inflammation, and suppression of immune responses.
Pharmacokinetics
Onset
Systemic effects usually begin within 6-48 hours after IM injection; local effects much faster.
Excretion
Mainly renal as inactive metabolites.
Half life
Biological half-life is 18-36 hours; plasma half-life is 2-3 hours.
Absorption
Well absorbed after intramuscular (IM) or intra-articular injection; absorption rate varies based on injection site and blood flow.
Metabolism
Primarily hepatic, metabolized by CYP3A4 enzymes.
Side Effects
Contraindications
- •Systemic fungal infections
- •Known hypersensitivity to methylprednisolone or any component of the formulation
- •Administration of live or live-attenuated vaccines during corticosteroid therapy
- •Cerebral malaria
- •Intrathecal administration
Drug Interactions
NSAIDs
Increased risk of gastrointestinal bleeding.
Live vaccines
Avoid during corticosteroid therapy due to risk of disseminated infection.
Anticoagulants (e.g., warfarin)
Concurrent use may modify anticoagulant effects; monitor INR.
Diuretics (e.g., thiazides, loop diuretics)
Enhanced potassium excretion, increased risk of hypokalemia.
CYP3A4 inducers (e.g., rifampicin, phenytoin, barbiturates)
May decrease methylprednisolone levels and effects.
CYP3A4 inhibitors (e.g., ketoconazole, macrolide antibiotics)
May increase methylprednisolone levels and effects.
Storage
Store at controlled room temperature (20°C to 25°C), protect from freezing. Do not refrigerate. Shake well before use.
Overdose
Acute overdose is rare and usually not life-threatening. Chronic overdose can lead to severe side effects like Cushing's syndrome, adrenal suppression, hyperglycemia. Treatment is symptomatic and supportive; gradual withdrawal if chronic.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Methylprednisolone is excreted in breast milk; caution advised, weigh risks vs. benefits, or consider discontinuing breastfeeding.
