Description
Overview of the medicine
Dermasol Plus Cream is a combination medicine used to treat inflammatory skin conditions where bacterial or fungal infections are present or suspected. It contains a potent corticosteroid (Clobetasol Propionate) to reduce inflammation, an antibiotic (Neomycin Sulfate) to fight bacterial infections, and an antifungal (Miconazole Nitrate) to combat fungal infections.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area 2-3 times daily, or as directed by a physician. Treatment duration should be limited, usually not exceeding 7-14 days.
Elderly
No specific dose adjustment is required, but use with caution due to potential for thinner skin.
Renal_impairment
Minimal systemic absorption, so dose adjustment is generally not necessary.
How to Take
For external topical use only. Apply a small amount to the affected area and rub in gently. Wash hands before and after application.
Mechanism of Action
Clobetasol Propionate reduces inflammation, itching, and redness by inducing phospholipase A2 inhibitory proteins (lipocortins). Neomycin Sulfate inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit. Miconazole Nitrate inhibits ergosterol synthesis, impairing fungal cell membrane function.
Pharmacokinetics
Onset
Symptom relief typically within a few days of initiation.
Excretion
Absorbed components are primarily excreted via the kidneys.
Half life
Systemic half-life not primarily relevant for topical use; local effects persist for several hours.
Absorption
Minimal systemic absorption through intact skin. Absorption may increase with occlusive dressings, extensive application, or on damaged skin.
Metabolism
Absorbed components are primarily metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to Clobetasol Propionate, Neomycin Sulfate, Miconazole Nitrate, or any component of the cream.
- Viral skin infections (e.g., herpes simplex, varicella).
- Tuberculosis of the skin, acne rosacea, perioral dermatitis, widespread plaque psoriasis.
- Not for use in children under 2 years of age.
Drug Interactions
0
Due to minimal systemic absorption, clinically significant drug interactions are rare with topical application. However, prolonged use over large areas may theoretically increase systemic absorption of clobetasol, leading to potential interactions with drugs affecting CYP3A4 enzymes.
Storage
Store below 30°C. Do not freeze. Protect from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is unlikely with topical application. Chronic overdose or misuse, especially over large areas or under occlusive dressings, can lead to systemic effects of corticosteroids (e.g., Cushing's syndrome, adrenal suppression) or local adverse effects like skin atrophy.
Pregnancy & Lactation
Pregnancy Category C. Use with caution during pregnancy only if the potential benefit justifies the potential risk to the fetus. Avoid extensive or prolonged use. For lactating mothers, use with caution; it is unknown if components are excreted in human milk after topical application. Avoid applying to the breast area.
