Dermatas N Cream is a combination topical medication containing Betamethasone Valerate, a potent corticosteroid, and Neomycin Sulfate, a broad-spectrum antibiotic. It is used to treat inflammatory skin conditions such as eczema and psoriasis that are complicated by bacterial infections or are at risk of secondary infection.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin area 2-3 times daily. Gently rub in. Treatment should not exceed 7 days without medical review.
Elderly
Same as adults, but caution should be exercised with prolonged use due to increased risk of skin thinning and absorption.
Renal_impairment
No specific dosage adjustment is generally required for topical use due to minimal systemic absorption. However, caution is advised in severe impairment if extensive application occurs.
How to Take
For external use only. Apply a thin layer to the affected skin area. Wash hands before and after application. Avoid contact with eyes, mouth, and other mucous membranes. Do not apply to open wounds or broken skin.
Mechanism of Action
Betamethasone, a corticosteroid, acts by inducing phospholipase A2 inhibitory proteins, thereby inhibiting the release of arachidonic acid and subsequently the formation of inflammatory mediators like prostaglandins and leukotrienes. Neomycin, an aminoglycoside antibiotic, binds to the 30S ribosomal subunit of susceptible bacteria, inhibiting protein synthesis and leading to bacterial cell death.

Excretion
Absorbed Betamethasone metabolites are excreted primarily via the kidneys. Absorbed Neomycin is excreted largely unchanged by the kidneys.
Half life
Not precisely defined for topical application. Systemic Betamethasone has a biological half-life of 3-5 hours. Systemic Neomycin has a serum half-life of 2-3 hours.
Absorption
Limited systemic absorption, especially through intact skin. Absorption may increase with occlusion, inflammation, or large surface areas. Neomycin absorption is minimal.
Metabolism
Betamethasone is primarily metabolized in the liver. Neomycin is not significantly metabolized.

Contraindications
- Hypersensitivity to Betamethasone, Neomycin, or any other aminoglycoside antibiotics or excipients.

Although systemic absorption of topical neomycin is minimal, concurrent use with systemic aminoglycosides could theoretically increase the risk of ototoxicity or nephrotoxicity, especially with extensive application to broken skin.
Storage
Store below 25°C. Protect from light and moisture. Do not freeze. Keep out of reach of children. Do not use after the expiry date.
Overdose
Systemic overdose is unlikely with appropriate topical use. Prolonged and extensive use may lead to systemic corticosteroid effects (e.g., adrenal suppression, Cushing's syndrome) or, rarely, neomycin-related ototoxicity or nephrotoxicity. Treatment is symptomatic and supportive; gradual withdrawal of the corticosteroid is advised if systemic effects occur.
