Description
Overview of the medicine
Dermex-NN 0.05 mg Cream is a topical preparation combining a potent corticosteroid (Clobetasol Propionate), an aminoglycoside antibiotic (Neomycin Sulfate), and an antifungal agent (Nystatin). It is used to treat various inflammatory skin conditions that are complicated by fungal and/or bacterial infections.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin area once or twice daily. Do not use for more than 2-4 weeks continuously without medical advice.
Elderly
Same as adult dosage, but use with caution due to thinner skin and potential for increased absorption.
Renal_impairment
No specific dosage adjustment required for topical application, but caution advised if extensive skin areas are treated due to potential systemic absorption of neomycin.
How to Take
For external use only. Wash hands before and after applying the cream. Apply a thin layer to the affected area and gently rub in. Do not cover with occlusive dressings unless advised by a doctor.
Mechanism of Action
Clobetasol Propionate exerts anti-inflammatory, antipruritic, and vasoconstrictive actions. Neomycin Sulfate is an aminoglycoside antibiotic that inhibits bacterial protein synthesis. Nystatin is a polyene antifungal that binds to sterols in the fungal cell membrane, altering its permeability and causing cell leakage.
Pharmacokinetics
Onset
Local effects usually observed within a few days of application.
Excretion
Primarily excreted via the kidneys and bile for systemically absorbed components.
Half life
Not precisely determined for topical application due to minimal systemic absorption; for absorbed Clobetasol, plasma half-life is approximately 3-5 hours.
Absorption
Minimal systemic absorption through intact skin; absorption is increased in areas of skin damage, inflammation, or with occlusive dressings.
Metabolism
Absorbed Clobetasol is metabolized primarily in the liver.
Side Effects
Contraindications
- •Hypersensitivity to Clobetasol Propionate, Neomycin, Nystatin, or any component of the cream
- •Viral infections of the skin (e.g., herpes simplex, varicella)
- •Tuberculous or syphilitic lesions of the skin
- •Acne vulgaris, rosacea, perioral dermatitis
- •Vaccination reactions
- •Extensive and/or deep skin wounds
Drug Interactions
Aminoglycosides (systemic)
Although systemic absorption is minimal, prolonged use over large areas could theoretically lead to additive ototoxicity/nephrotoxicity if systemic aminoglycosides are also used.
Other topical corticosteroids
Concurrent use of other topical corticosteroids should be avoided as it may increase systemic absorption and adverse effects.
Storage
Store below 30°C in a dry place, away from light. Do not freeze. Keep out of reach of children.
Overdose
Topical overdose is unlikely to produce acute systemic effects. However, prolonged or excessive use may lead to systemic corticosteroid effects (e.g., Cushing's syndrome) or ototoxicity/nephrotoxicity from neomycin, especially with extensive skin application or occlusive dressings. Treatment is symptomatic and supportive; discontinue the cream.
Pregnancy & Lactation
Use during pregnancy and lactation only if the potential benefit outweighs the potential risk to the fetus or infant. Avoid extensive use or application to large surface areas. Consult a doctor.
