Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area once or twice daily. Do not use for more than 2-4 weeks without medical review.
Elderly
Use with caution. Start with once daily application and monitor for adverse effects due to thinning skin.
Renal_impairment
No specific dosage adjustment required for topical application, but caution if large areas are treated or prolonged use.
How to Take
For topical use only. Apply a thin film to the affected skin area and rub in gently. Wash hands before and after application. Avoid contact with eyes, mouth, and other mucous membranes.
Mechanism of Action
Clobetasol Propionate is a highly potent corticosteroid that provides anti-inflammatory, antipruritic, and vasoconstrictive actions. It acts by inducing phospholipase A2 inhibitory proteins, thereby controlling the biosynthesis of potent mediators of inflammation like prostaglandins and leukotrienes. Miconazole Nitrate is an azole antifungal that inhibits the biosynthesis of ergosterol, an essential component of the fungal cell membrane, leading to increased permeability and leakage of cellular contents, resulting in fungal cell death.
Pharmacokinetics
Onset
Anti-inflammatory and antipruritic effects of Clobetasol usually observed within a few days. Antifungal effects of Miconazole begin within days, with clinical improvement typically seen within 1-2 weeks.
Excretion
Metabolites of Clobetasol are excreted primarily by the kidneys and to a lesser extent in the bile. Miconazole metabolites are excreted in urine and feces.
Half life
Systemic half-life of absorbed Clobetasol is approximately 3-5 hours. Miconazole's systemic half-life after topical application is negligible due to minimal absorption.
Absorption
Systemic absorption of Clobetasol Propionate varies depending on the vehicle, integrity of the epidermal barrier, and use of occlusive dressings; approximately 0.05% to 2.5% can be absorbed. Miconazole Nitrate is minimally absorbed systemically following topical application.
Metabolism
Absorbed Clobetasol is metabolized primarily in the liver. Miconazole, if systemically absorbed, is also metabolized in the liver.
Side Effects
Contraindications
- •Hypersensitivity to Clobetasol Propionate, Miconazole Nitrate, or any component of the formulation.
- •Primary cutaneous viral infections (e.g., herpes simplex, varicella).
- •Primary bacterial infections unless used concomitantly with appropriate anti-infective therapy.
- •Perioral dermatitis, acne vulgaris, rosacea, perianal and genital pruritus.
- •Not for use in children under 1 year of age.
Drug Interactions
Warfarin
Topical Miconazole, though minimally absorbed, may enhance anticoagulant effects of Warfarin in rare cases. Monitor INR.
Systemic Corticosteroids
Increased risk of systemic corticosteroid adverse effects if significant absorption occurs.
Storage
Store below 30°C. Do not freeze. Keep out of reach of children.
Overdose
Acute overdose is unlikely with topical application. Chronic overdose or misuse can lead to signs of hypercorticism (Cushing's syndrome), HPA axis suppression, and local skin atrophy. Treatment is symptomatic and supportive; gradual withdrawal of the corticosteroid may be necessary.
