Description
Overview of the medicine
Dermova 0.05% Ointment contains Clobetasol Propionate, a very potent corticosteroid, used for the short-term treatment of severe inflammatory skin conditions that do not respond adequately to less potent steroids.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area once or twice daily. Treatment should be limited to 2-4 weeks, or until control is achieved, if sooner. Do not exceed 50g per week.
Elderly
Use with caution due to increased risk of skin atrophy and systemic absorption. Dosage usually similar to adults, but monitor closely.
Renal_impairment
No specific dosage adjustment recommended, but use with caution in patients with significant renal impairment due to potential for increased systemic exposure.
How to Take
For external use only. Apply a thin film to the affected skin area and gently rub in, once or twice daily, as directed by a physician. Avoid contact with eyes.
Mechanism of Action
Clobetasol propionate is a highly potent topical corticosteroid. It exerts its anti-inflammatory, antipruritic, and vasoconstrictive actions by inducing phospholipase A2 inhibitory proteins, thereby controlling the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes.
Pharmacokinetics
Onset
Effect observed within hours of application.
Excretion
Excreted mainly via the kidneys, with some biliary excretion.
Half life
Systemic half-life is approximately 3-5 hours after absorption.
Absorption
Variable systemic absorption occurs, depending on the extent of skin involvement, integrity of the epidermal barrier, and use of occlusive dressings. Absorption is increased on areas like the face, groin, and axillae.
Metabolism
Primarily metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to Clobetasol Propionate or any component of the ointment.
- Rosacea, acne vulgaris, perioral dermatitis, pruritus without inflammation.
- Perianal and genital pruritus.
- Primary cutaneous viral infections (e.g., herpes simplex, chickenpox).
- Primary fungal or bacterial infections of the skin.
- Children under 12 years of age.
Drug Interactions
Ritonavir, Itraconazole
May increase systemic exposure to clobetasol propionate, increasing the risk of systemic side effects.
Storage
Store below 30°C. Protect from light and moisture. Keep out of the reach of children.
Overdose
Acute overdose is unlikely due to topical application. In case of chronic overdose or misuse, symptoms of hypercorticism (Cushing's syndrome) may appear, requiring gradual withdrawal under medical supervision.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Use during lactation should be with caution; the benefits of breastfeeding should be weighed against the potential for infant exposure.

Lab Monitoring
- Monitoring of HPA axis suppression (e.g., cortisol levels) may be necessary for patients using the ointment extensively or for prolonged periods, especially in children.
Doctor Notes
- Educate patients on proper application technique and duration limits to minimize systemic absorption and local side effects.
- Advise against use on the face, intertriginous areas, and children unless absolutely necessary and under strict supervision.
- Monitor for signs of HPA axis suppression in high-risk patients (e.g., extensive body surface area, occlusive dressings, prolonged use).
Patient Guidelines
- Use only as directed by your doctor. Do not use for longer than the prescribed period.
- Wash your hands before and after applying the ointment.
- Avoid applying to the face, groin, or underarms unless specifically instructed by your doctor.
- Do not use occlusive dressings (e.g., bandages) over the treated area unless directed by your doctor, as this can increase absorption.
