Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor that decreases the amount of acid produced in the stomach. It is used to treat conditions caused by too much stomach acid, such as gastroesophageal reflux disease (GERD), stomach ulcers, and to help heal a damaged esophagus.
Uses & Indications
Dosage
Adults
For GERD: 20 mg or 40 mg once daily for 4 to 8 weeks. For Erosive Esophagitis: 20 mg or 40 mg once daily for 4 to 8 weeks. For H. pylori eradication: 40 mg once daily for 10 days in combination with amoxicillin and clarithromycin.
Elderly
No specific dosage adjustment is generally required for elderly patients based on age alone.
Renal_impairment
No dosage adjustment is generally required for patients with renal impairment.
How to Take
Take orally at least one hour before a meal. Swallow the capsule whole with water. Do not chew, crush, or open the capsule.
Mechanism of Action
Esomeprazole selectively inhibits the gastric H+/K+-ATPase (proton pump) in the parietal cells of the stomach, thereby blocking the final step of acid secretion and reducing gastric acid production.
Pharmacokinetics
Onset
Within 1 hour of administration.
Excretion
Eliminated primarily by the kidneys (approximately 80%) and in feces (approximately 20%).
Half life
Approximately 1 to 1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 64% after a single 40 mg dose and increases to 89% with repeated once-daily administration.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, mainly by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Diazepam
May decrease the metabolism of diazepam, increasing its concentration and potentially leading to toxicity.
Warfarin
Monitor INR carefully due to potential for increased bleeding risk.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel by inhibiting its conversion to its active metabolite. Avoid concomitant use.
Methotrexate
May increase methotrexate levels and prolong its elimination, potentially leading to methotrexate toxicity. Consider temporary discontinuation of esomeprazole.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Experience with esomeprazole overdose is limited. No specific antidote is known. Management should be symptomatic and supportive.
