Description
Overview of the medicine
Desloratadine is a non-sedating antihistamine used to relieve symptoms of allergic rhinitis and chronic idiopathic urticaria. It helps to alleviate sneezing, runny nose, itchy nose or eyes, congestion, and skin itching and hives.
Uses & Indications
Dosage
Adults
5 mg (one tablet) once daily.
Elderly
Generally, no dose adjustment is necessary, but monitor renal function. For patients with renal or hepatic impairment, consider alternate day dosing.
Renal_impairment
For severe renal impairment (creatinine clearance < 30 mL/min), 5 mg every other day. Adjust dose based on individual response and tolerability.
How to Take
Desloratadine tablet can be taken orally, with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Desloratadine is a potent, selective, peripheral H1-receptor antagonist. It prevents the binding of histamine to H1 receptors, thereby inhibiting the effects of histamine and reducing allergic symptoms like sneezing, itching, and rhinorrhea. It also has anti-inflammatory effects.

Primarily excreted via urine (40%) and feces (40%) as metabolites.
Half life
Approximately 27 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached in approximately 3 hours.
Metabolism
Extensively metabolized to 3-hydroxydesloratadine (active metabolite) by conjugation. Metabolism involves cytochrome P450 enzymes, though specific isoforms are not clearly identified and there's significant inter-individual variability.
Side Effects
Contraindications
- •Hypersensitivity to desloratadine, loratadine, or any component of the formulation.

Drug Interactions
Cimetidine
Co-administration with cimetidine led to increased desloratadine plasma levels, but no clinically significant adverse effects were observed.
Erythromycin
Co-administration with erythromycin led to increased desloratadine plasma levels, but no clinically significant adverse effects were observed.
Ketoconazole
Co-administration with ketoconazole led to increased desloratadine plasma levels, but no clinically significant adverse effects were observed.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In case of overdose, seek immediate medical attention. Symptoms may include somnolence, increased heart rate, and headache. Treatment is symptomatic and supportive. Desloratadine is not eliminated by hemodialysis.
Pregnancy & Lactation
Pregnancy Category C. Desloratadine should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk; therefore, it is not recommended for use during lactation.
