Description
Overview of the medicine
Devolac (Lactulose) is a synthetic disaccharide used to treat constipation and hepatic encephalopathy. It works by drawing water into the colon, softening stools, and promoting bowel movements. In hepatic encephalopathy, it reduces ammonia levels in the blood.
Uses & Indications
Dosage
Adults
Constipation: Initially 15-45 ml daily, then adjust to 15-30 ml daily. Hepatic encephalopathy: Initially 30-45 ml three to four times daily, adjust to produce 2-3 soft stools per day.
Elderly
Similar to adult dosage, but caution with fluid/electrolyte balance.
Renal_impairment
No specific dose adjustment required as it's minimally absorbed.
How to Take
Oral. Can be taken with water or fruit juice. Dosage for constipation usually once daily, for hepatic encephalopathy divided doses.
Mechanism of Action
Lactulose is a non-absorbable disaccharide that passes through the stomach and small intestine largely unchanged. In the colon, it is metabolized by colonic bacteria into low molecular weight organic acids (lactic acid, acetic acid). This leads to an osmotic effect, drawing water into the colon, which softens the stool and increases its bulk, stimulating peristalsis. In hepatic encephalopathy, the acidification of the colon by these organic acids converts ammonia (NH3) to ammonium ion (NH4+), which is not absorbed and is expelled in the feces, thus reducing blood ammonia levels.
Pharmacokinetics
Onset
24-48 hours for laxative effect; several days for hepatic encephalopathy.
Excretion
Unmetabolized drug and its metabolites are excreted in feces. Minimal absorbed drug is excreted renally.
Half life
Not applicable as it's minimally absorbed. Unabsorbed drug is metabolized by colonic bacteria.
Absorption
Minimally absorbed (<3%) from the GI tract.
Metabolism
Metabolized by colonic bacteria into lactic acid, acetic acid, and other organic acids.
Side Effects
Contraindications
- Galactosemia
- Acute abdominal pain (e.g., appendicitis, intestinal obstruction) without diagnosis
- Known hypersensitivity to lactulose or any component
Drug Interactions
Antacids
May inhibit the desired pH drop in the colon, reducing the effect in hepatic encephalopathy.
Neomycin
May interfere with the degradation of lactulose by colonic bacteria, reducing its efficacy.
Non-absorbable antacids
May inhibit the desired pH drop in the colon.
Storage
Store below 30°C. Do not freeze. Keep out of reach of children.
Overdose
Symptoms: Diarrhea, abdominal pain, electrolyte disturbances (especially hypokalemia, hyponatremia). Management: Discontinue lactulose, correct fluid and electrolyte imbalances. Symptomatic and supportive treatment.
Pregnancy & Lactation
Considered safe during pregnancy and lactation as it is minimally absorbed.
