Overview of the medicine
Devomit 25 mg tablet contains Promethazine, which is an antihistamine with strong sedative, antiemetic (anti-sickness), and anticholinergic properties. It is used to relieve symptoms of nausea, vomiting, motion sickness, and also as a sedative.
Uses & Indications
Dosage
Adults
For nausea/vomiting: 25 mg as a single dose, then 12.5-25 mg every 4-6 hours if needed. For motion sickness: 25 mg 30-60 minutes before travel, repeated 8-12 hours later if necessary. For sedation: 25-50 mg at bedtime.
Elderly
Lower doses may be required due to increased sensitivity to anticholinergic and sedative effects. Start with 12.5 mg at bedtime for sedation.
Renal_impairment
No specific dose adjustment recommendations, but use with caution due to potential for accumulation of metabolites.
How to Take
Take orally, with or without food. To minimize stomach upset, it may be taken with food or milk.
Mechanism of Action
Promethazine acts as an H1-receptor antagonist, blocking histamine's effects. Its antiemetic effect is primarily due to its antagonism of dopamine D2 receptors in the chemoreceptor trigger zone (CTZ) and anticholinergic effects. Contraindications

Drug Interactions
Epinephrine
Promethazine may reverse the pressor effect of epinephrine, leading to a further drop in blood pressure.
MAO Inhibitors
May prolong and intensify the anticholinergic effects of promethazine.
CNS Depressants (e.g., alcohol, opioids, benzodiazepines)
Increased CNS depression, leading to increased sedation, respiratory depression, and hypotension.

Overdose
Symptoms of overdose include severe drowsiness, confusion, disorientation, severe dry mouth, blurred vision, dilated pupils, fever, hallucinations, convulsions, and respiratory depression. Management involves supportive care, maintaining airway, and treating symptoms.
Pregnancy & Lactation
Pregnancy Category C. Not recommended for use during late pregnancy or labor as it may cause respiratory depression in the newborn. Avoid during lactation as promethazine may pass into breast milk and cause drowsiness or irritability in the infant.
