Overview of the medicine
Dexaguard-01-05 Eye Drop is a combination ophthalmic solution containing a corticosteroid (Dexamethasone) and an aminoglycoside antibiotic (Tobramycin), used to treat inflammatory ocular conditions where bacterial infection or risk of bacterial infection exists.
Uses & Indications
Dosage
Adults
Instill 1-2 drops into the conjunctival sac of the affected eye(s) every 4-6 hours. For severe cases, dosage may be increased to 1-2 drops every 2 hours, then tapered as improvement occurs.
Elderly
No specific dosage adjustment required for elderly patients.
Renal_impairment
No specific dosage adjustment required for renal impairment as systemic absorption is minimal.
How to Take
For ophthalmic use only. Wash hands thoroughly before administration. Tilt head back, pull down the lower eyelid to create a pouch, and instill the drops. Avoid touching the dropper tip to the eye or any other surface. Close the eye gently for 1-2 minutes. If using other eye medications, wait at least 5 minutes between applications.
Mechanism of Action
Dexamethasone suppresses inflammatory responses by inhibiting phospholipase A2, thereby reducing prostaglandin and leukotriene formation. Tobramycin is an aminoglycoside antibiotic that inhibits bacterial protein synthesis by irreversibly binding to the 30S ribosomal subunit, leading to bacterial cell death.

Drug Interactions
Systemic corticosteroids
Potentiates systemic effects; however, systemic absorption from eye drops is minimal.
Other topical ophthalmic antibiotics
Avoid concurrent use unless specifically advised by a physician.
Other topical ophthalmic corticosteroids
Increased risk of ocular side effects (e.g., elevated IOP).
Storage
Store at room temperature (below 30°C). Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Topical ophthalmic overdose is unlikely to cause acute systemic toxicity due to minimal absorption. If excessive amount is instilled, flush the eye with lukewarm water. Symptomatic and supportive measures should be taken if accidental ingestion occurs.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs potential risk to the fetus. Dexamethasone and Tobramycin can be excreted in human milk. Caution should be exercised when administered to a nursing mother.
