Description
Overview of the medicine
Dexcilo-03-01-eye-drop is a combination eye drop containing Moxifloxacin, a broad-spectrum fluoroquinolone antibiotic, and Dexamethasone, a potent corticosteroid. It is used to treat bacterial eye infections where inflammation is also present.
Uses & Indications
Dosage
Adults
Instill 1 drop into the affected eye(s) 2 to 4 times daily, or as directed by a physician. In severe cases, dosage may be increased to 1 drop every 2 hours for the first 2 days, then decreased.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No specific dosage adjustment is required as systemic absorption is minimal.
How to Take
For topical ophthalmic use only. Wash hands before use. Tilt head back, pull down the lower eyelid to form a pouch, instill the drop, and close the eye gently. Do not touch the dropper tip to any surface. Apply pressure to the tear duct for a minute to minimize systemic absorption.
Mechanism of Action
Moxifloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes for bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death. Dexamethasone is a corticosteroid that suppresses the inflammatory response by inhibiting multiple inflammatory mediators, reducing swelling, redness, and pain.
Pharmacokinetics
Onset
Onset of action within minutes to hours for anti-inflammatory effects; antibacterial effects evident within hours.
Excretion
Mainly through ocular drainage; systemically absorbed amounts are excreted via urine and feces.
Half life
Local half-life in ocular tissues varies, systemic half-life of absorbed components is typically longer but clinically insignificant due to low absorption.
Absorption
Minimal systemic absorption after topical ophthalmic administration. Therapeutic concentrations are achieved in ocular tissues.
Metabolism
Minimal metabolism occurs in ocular tissues; systemically absorbed small amounts are metabolized in the liver.
Side Effects
Contraindications
- Hypersensitivity to Moxifloxacin, Dexamethasone, or any other component of the formulation.
- Viral diseases of the cornea and conjunctiva, including epithelial herpes simplex keratitis (dendritic keratitis), vaccinia, and varicella.
- Mycobacterial infection of the eye.
- Fungal diseases of ocular structures.
Drug Interactions
Systemic drugs
Systemic interactions are unlikely due to minimal systemic absorption.
Other ophthalmic preparations
If using more than one topical ophthalmic product, administer them at least 5 minutes apart to avoid washout.
Storage
Store at room temperature (below 30°C). Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Overdose with topical ophthalmic administration is unlikely. If excessive amounts are accidentally instilled, the eye may be rinsed with lukewarm water.
Pregnancy & Lactation
Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Dexamethasone may pass into breast milk; use with caution in nursing mothers. Consult a doctor.
