Description
Overview of the medicine
Dextromethorphan is a cough suppressant used for the temporary relief of cough caused by the common cold, flu, or other inhaled irritants. It works by decreasing activity in the part of the brain that causes coughing.
Uses & Indications
Dosage
Adults
10-20 mg (5-10 mL) every 4 hours or 30 mg (15 mL) every 6-8 hours, not to exceed 120 mg in 24 hours.
Elderly
Same as adult dose, but caution is advised in patients with impaired renal or hepatic function.
Renal_impairment
Use with caution; dose adjustment may be necessary in severe impairment.
How to Take
Oral. May be taken with or without food. Use a measuring spoon or cup for accurate dosing.
Mechanism of Action
Dextromethorphan is a cough suppressant that acts centrally on the cough center in the medulla, elevating the cough threshold. It is an N-methyl-D-aspartate (NMDA) receptor antagonist.

Onset
15-30 minutes.
Excretion
Primarily excreted in urine as metabolites.
Half life
1.4-3.9 hours (for parent drug); active metabolite (dextrorphan) 11 hours.
Absorption
Rapidly absorbed from the GI tract. Peak plasma levels in 2-2.5 hours.
Metabolism
Extensively metabolized by hepatic cytochrome P450 2D6 (CYP2D6) to dextrorphan (active metabolite).
Side Effects
Contraindications
- •Hypersensitivity to dextromethorphan or any component
- •Concurrent use with MAO inhibitors or within 14 days of stopping MAO inhibitors
Drug Interactions
Alcohol
Increased CNS depression.
SSRIs/SNRIs
Increased risk of serotonin syndrome.
MAO Inhibitors
Increased risk of serotonin syndrome, hyperpyrexia, coma.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, dizziness, drowsiness, blurred vision, nystagmus, lethargy, ataxia, stupor, hallucinations, respiratory depression, coma. Treatment is symptomatic and supportive. Naloxone may be used in severe cases.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed. Consult doctor. Small amounts may pass into breast milk; use with caution in nursing mothers.
