Description
Overview of the medicine
Dextrop 10 mg syrup is an antitussive medication containing Dextromethorphan Hydrobromide, used to temporarily relieve coughs caused by minor throat and bronchial irritation, common cold, or flu.
Uses & Indications
Dosage
Adults
10-20 mg (1-2 teaspoons) every 4 hours, or 30 mg (3 teaspoons) every 6-8 hours, not to exceed 120 mg in 24 hours.
Elderly
Same as adult dosage, but caution with other medications.
Renal_impairment
Use with caution. Dosage reduction may be necessary in severe impairment.
How to Take
Oral. May be taken with or without food. Use measuring spoon/cup provided for accurate dosing.
Mechanism of Action
Dextromethorphan acts centrally on the cough center in the medulla oblongata to suppress the cough reflex. It elevates the threshold for coughing without affecting ciliary activity or mucus expectoration.
Pharmacokinetics
Onset
15-30 minutes
Excretion
Excreted primarily via the kidneys as unchanged drug and metabolites.
Half life
2-4 hours (for extensive metabolizers), up to 15-30 hours (for poor metabolizers).
Absorption
Well absorbed from the gastrointestinal tract.
Metabolism
Extensively metabolized in the liver by CYP2D6 to its active metabolite, dextrorphan, and other metabolites.
Side Effects
Contraindications
- Hypersensitivity to dextromethorphan or any component of the formulation.
- Concurrent use with MAOIs (Monoamine Oxidase Inhibitors) or within 14 days of discontinuing MAOIs.
- Children under 4 years of age (generally for cough/cold products).
Drug Interactions
Alcohol and other CNS depressants
May increase CNS depression.
MAOIs (Monoamine Oxidase Inhibitors)
Concomitant use can lead to serotonin syndrome, a potentially life-threatening reaction.
SSRIs (Selective Serotonin Reuptake Inhibitors)
Increased risk of serotonin syndrome.
Storage
Store at room temperature (below 30°C/86°F), away from direct light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, dizziness, drowsiness, blurred vision, nystagmus, ataxia, respiratory depression, and serotonin syndrome. Treatment is supportive, potentially including activated charcoal, naloxone (for severe cases of respiratory depression), and serotonin antagonists.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Small amounts may pass into breast milk; use with caution during lactation and consult a doctor.
