Description
Overview of the medicine
Diactin 5 mg tablet contains Cyproheptadine Hydrochloride, an antihistamine with antiserotonergic and anticholinergic properties. It is used to relieve symptoms of various allergic conditions and also as an appetite stimulant.
Uses & Indications
Dosage
Adults
For allergies: Initial 4 mg three times daily (TID), maximum 0.5 mg/kg/day or 32 mg/day, whichever is less. For appetite stimulation: 4 mg three times daily.
Elderly
Start with lower doses (e.g., 4 mg once or twice daily) due to increased sensitivity and potential for adverse effects. Dose adjustment based on renal and hepatic function.
Renal_impairment
Use with caution; dose reduction may be necessary in severe impairment. Monitor renal function closely.
How to Take
Diactin 5 mg tablet is for oral administration. It can be taken with or without food. Taking it with food may help reduce gastrointestinal upset.
Mechanism of Action
Cyproheptadine acts as a potent H1-receptor antagonist, blocking the effects of histamine and thus reducing allergic symptoms. It also possesses antiserotonergic activity by blocking 5-HT2 receptors, which is thought to contribute to its appetite-stimulating effects.
Pharmacokinetics
Onset
Antihistamine effects usually begin within 15-30 minutes. Appetite stimulating effects may take longer and vary.
Excretion
Mainly excreted in urine (about 40%) and feces (about 2-20%) as metabolites.
Half life
Approximately 16 hours.
Absorption
Well absorbed from the gastrointestinal tract. Peak plasma concentrations are usually reached within 6-9 hours after administration.
Metabolism
Extensively metabolized in the liver, primarily via glucuronidation.

- Newborns or premature infants
- Breastfeeding mothers
- Angle-closure glaucoma
- Stenosing peptic ulcer
- Symptomatic prostatic hypertrophy
- Bladder neck obstruction
- Concomitant use with Monoamine Oxidase (MAO) inhibitors
Drug Interactions
MAO Inhibitors
May prolong and intensify the anticholinergic effects of antihistamines (e.g., dry mouth, urinary retention).
CNS Depressants
Additive sedative effects may occur with alcohol, sedatives, hypnotics, anxiolytics, and other CNS depressants.
Anticholinergic drugs
May enhance anticholinergic effects when co-administered with other anticholinergic agents.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, dizziness, ataxia, blurred vision, dry mouth, urinary retention, dilated pupils, and in severe cases, CNS depression or stimulation, convulsions, respiratory depression, and coma. Treatment is symptomatic and supportive; gastric lavage or activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category B (US FDA). Use only if clearly needed and the potential benefits outweigh the potential risks. Not recommended during breastfeeding as it may cause sedation in infants and potentially decrease milk supply.
