Overview of the medicine
Diafre 500 mg Tablet contains Metformin Hydrochloride, an oral antidiabetic drug used to manage type 2 diabetes mellitus, especially in overweight patients, when diet and exercise alone do not provide adequate glycemic control. It helps to lower blood glucose levels by decreasing glucose production in the liver, decreasing intestinal absorption of glucose, and improving insulin sensitivity.
Uses & Indications
Dosage
Adults
Initial dose: 500 mg once or twice daily with meals. May be gradually increased to a maximum of 2000-2500 mg daily in divided doses. Taken with or after food to minimize gastrointestinal side effects.
Elderly
Lower initial doses may be considered, and renal function should be closely monitored.
Renal_impairment
Not recommended for eGFR < 30 mL/min/1.73 m². Dose reduction or discontinuation may be necessary for eGFR between 30-60 mL/min/1.73 m².
How to Take
Take Diafre 500 mg Tablet orally, preferably with or after meals to reduce gastrointestinal discomfort. Swallow the tablet whole with a glass of water, do not crush or chew it.
Mechanism of Action
Metformin primarily reduces hepatic glucose production by inhibiting gluconeogenesis and glycogenolysis. It also increases insulin sensitivity, enhancing peripheral glucose uptake and utilization in muscle cells. Additionally, it delays intestinal glucose absorption. Unlike sulfonylureas, it does not stimulate insulin secretion and therefore does not cause hypoglycemia in monotherapy.

Excretion
Excreted unchanged in the urine; renal elimination is the primary route.
Half life
Plasma elimination half-life is approximately 6.2 hours.
Absorption
Oral bioavailability is 50-60% under fasting conditions. Food decreases the extent and slightly delays the absorption.
Metabolism
Metformin is not metabolized in the liver.
Side Effects
Contraindications
- •Hypersensitivity to metformin or any excipients.
- •Severe renal impairment (eGFR < 30 mL/min/1.73 m²).
- •Acute or chronic metabolic acidosis, including diabetic ketoacidosis.
- •Dehydration, severe infection, shock.
- •Hepatic impairment, acute alcohol intoxication.
Drug Interactions
Alcohol
Increases the risk of lactic acidosis.
Iodinated Contrast Agents
Increased risk of lactic acidosis; discontinue metformin before or at the time of procedure and restart 48 hours later after assessing renal function.
Carbonic Anhydrase Inhibitors (e.g., Topiramate, Acetazolamide)
May increase the risk of lactic acidosis.
Cationic Drugs (e.g., Amiloride, Digoxin, Morphin, Procainamide, Quinidine, Quinine, Ranitidine, Triamterene, Trimethoprim, Vancomycin)
May compete with metformin for renal tubular transport, potentially increasing metformin plasma concentrations.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose of metformin can lead to lactic acidosis, especially in patients with predisposing factors such as renal impairment. Symptoms include malaise, myalgia, respiratory distress, somnolence, and abdominal distress. Treatment is primarily supportive; hemodialysis is the most effective method for removing lactate and metformin from the blood.
Pregnancy & Lactation
Use during pregnancy is generally not recommended, insulin is preferred. Consult your doctor. Metformin is excreted into breast milk; however, the amount is usually low and considered compatible with breastfeeding. Discuss with your healthcare provider.
