Description
Overview of the medicine
Gliclazide is an oral hypoglycemic agent of the sulfonylurea class used in the management of type 2 diabetes mellitus. It helps to lower blood glucose levels by stimulating insulin release from the pancreatic beta cells.
Uses & Indications
Dosage
Adults
Initially 40-80 mg daily, taken with breakfast. Adjusted according to response, usually 80-320 mg daily in divided doses. Doses over 160 mg usually given twice daily.
Elderly
Similar to adult dosage, but caution with initial dose and titration due to increased risk of hypoglycemia.
Renal_impairment
Mild to moderate renal impairment may require lower initial doses and careful monitoring. Not recommended for severe renal impairment.
How to Take
Oral, usually taken with food, preferably breakfast, to reduce the risk of gastrointestinal upset and ensure optimal absorption.
Mechanism of Action
Gliclazide binds to sulfonylurea receptors on pancreatic beta-cells, leading to depolarization, which opens voltage-gated calcium channels. The resulting calcium influx stimulates insulin secretion. It also has hemovascular effects.

Excretion
Primarily via the kidneys as metabolites, with a small amount excreted in feces.
Half life
Approximately 10-12 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract. Bioavailability is about 97%.
Metabolism
Extensively metabolized in the liver by CYP2C9, primarily to inactive metabolites.

Contraindications
- Hypersensitivity to Gliclazide or other sulfonylureas
- Type 1 diabetes
- Diabetic ketoacidosis
- Severe renal or hepatic impairment
- Concomitant miconazole treatment
- Pregnancy and lactation
Drug Interactions
Miconazole
Potentiates the hypoglycemic effect, leading to severe hypoglycemia.
Corticosteroids, Diuretics, Estrogens, Progestogens
Can reduce the hypoglycemic effect.
Beta-blockers, ACE inhibitors, NSAIDs, Salicylates, Sulfonamides
Can enhance the hypoglycemic effect.

Store below 30°C in a dry place, away from light and moisture.
