Description
Overview of the medicine
Teneligliptin is an oral anti-diabetic drug used for the treatment of type 2 diabetes mellitus. It belongs to the class of dipeptidyl peptidase-4 (DPP-4) inhibitors. It helps to lower blood glucose levels by increasing the levels of incretin hormones.
Uses & Indications
Dosage
Adults
50 mg once daily, preferably in the morning, with or without food.
Elderly
No specific dose adjustment required based on age, but caution is advised in elderly patients with impaired renal function.
Renal_impairment
No dose adjustment required for mild to moderate renal impairment. For severe renal impairment or end-stage renal disease, monitoring is recommended.
How to Take
Orally, once daily, with or without food.
Mechanism of Action
Teneligliptin inhibits the enzyme dipeptidyl peptidase-4 (DPP-4), which is responsible for inactivating incretin hormones (GLP-1 and GIP). By inhibiting DPP-4, teneligliptin increases the levels of active incretin hormones, leading to enhanced glucose-dependent insulin secretion from pancreatic beta cells and reduced glucagon secretion from alpha cells, thereby lowering blood glucose levels.

Onset
Within hours of administration, sustained blood glucose lowering effect.
Excretion
Excreted via both renal and hepatic routes, with approximately 50% excreted unchanged in urine and feces.
Half life
Approximately 24 hours, allowing for once-daily dosing.
Absorption
Rapidly absorbed from the gastrointestinal tract. Peak plasma concentration reached in 1-2 hours. Bioavailability is high.
Metabolism
Primarily metabolized by cytochrome P450 enzymes (CYP3A4 and CYP2D6) and non-CYP pathways.

- Type 1 Diabetes Mellitus

Drug Interactions
Digoxin
May slightly increase digoxin levels, monitor digoxin concentration.
CYP3A4/2D6 inhibitors/inducers
Co-administration with strong inhibitors or inducers of CYP3A4 and CYP2D6 may alter teneligliptin exposure, though dose adjustment is generally not required.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, seek medical attention. Symptomatic and supportive treatment should be initiated. Teneligliptin is not dialyzable.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. It is not known whether teneligliptin is excreted in human milk; caution should be exercised when administered to a nursing woman.
